Job Description

• Night shift Registered Nurse for a critical access hospital
• Schedule: 3 shifts per week, 12 hours each
• Shift hours: 6:30 PM to 7:00 AM
• Work includes alternating weekends and holidays

Required Credentials & Onboarding Requirements

Licenses & Certifications

• Current nursing license with Nursys verification within 30 days prior to start
• License/Certificate primary source verification within 30 days prior to start
• BLS certification (AHA required)
• Valid Driver’s License or State ID (must not be expired and address must comply with compact license rules)

Exams & Testing (Must be completed prior to start and updated as required)

• Ascension Pharmacology Exam (minimum 80%, max 2 attempts)
• Ascension Medical Surgical Exam (minimum 80%, max 2 attempts)
• Ascension EKG Exam (minimum 80%, max 2 attempts; updated edition as of Dec 1, 2023)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, max 2 attempts)
• Ascension Skills Checklist (clinical specialty-specific; completed within 1 year and updated annually)

Health & Immunization Requirements

• MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
• Tdap: Valid within 10 years (no declinations accepted)
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Optional, but if declined during flu season (Oct 1 - Mar 31), mask wearing required
• Fit Test: Required for all travelers; preferred before start, can be completed onsite (annual update)
• TB Screening: 2-step PPD or IGRA within specified timeframes; if positive, chest X-ray and annual TB questionnaire required
• Health Statement: Statement of good health or physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen: Within 30 days prior to start and annually thereafter

Professional Documentation

• Ascension Resume/Profile
• At least two professional references prior to first assignment
• Criminal Background Check (7-year search including SSN trace, employment verification, OIG, sex offender registry; updated annually)
• EPLS check within 90 days of start
• Education Verification relevant to clinical work within 90 days of start
• Employment Verification for past 7 years, updated annually
• Confirmation of Assignment documentation upon initial assignment and extensions
• Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
• Ascension Nursing Contract Worker Onboarding (completed onsite)
• Ascension Badge Photo (forward-facing headshot with white or blue background, no filters)
