Required Credentials and Compliance for Onboarding

Licenses & Certifications

• BLS (Basic Life Support): AHA or ARC only
• ACLS (Advanced Cardiovascular Life Support): AHA only
• Nursys License Verification: Nursing license verification within 30 days of start
• Primary Source Verification: Professional license and/or certification verification within 30 days of start, must be active for Louisiana

Skills & Competency Testing

• FMOLHS Skills Checklist: Unit-specific, within 1 year prior to start, updated annually
• FMOLHS Competency Exam: Passing score of 80% or higher within 1 year prior to start
• FMOLHS Core Competencies: Includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; must be current within 1 year prior to start with passing score of 80% or higher, updated annually

Personal Health Requirements

• MMR: Proof of 2 immunizations, positive titer, or declination with signed statement
• Fit Test: N95 mask fit test (3M 1860, 1860s, or 1870+), completed within 1 year prior to start, updated annually
• Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required signatures
• TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test (1 within 30 days, 1 within 1 year); if positive, MD note or proof of treatment required
• Health Statement: Signed and dated within 1 year of start, stating ability to work without restrictions
• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap: Proof of vaccination within past 10 years or declination with required signatures
• Annual TB Questionnaire: Required within 6 months of start; if positive, documentation of evaluation and treatment completion required
• FMOLHS Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start

Optional Personal Health

• COVID-19 Vaccine: Proof or exemption accepted
• Influenza Vaccine: Recommended; medical or religious exemptions accepted with proper documentation

Professional Requirements

• Resume: Required for submission
• Criminal Background Check: Completed within 30 days prior to start, including SSN address trace, county criminal searches for all counties lived/worked in last 7 years, national criminal check, education and employment verification (last 5 years), driving record
• Drug Screen: Negative 10-panel drug screen within 30 days prior to start
• HRIS Form: Required for submission
• Badge Photo: Forward-facing, well-lit, neutral background, JPEG format
• Contract Annual Education: Required as per document library
• Code of Conduct: Signed and dated annually
