Job Requirements

• Minimum of Certified Respiratory Therapist (CRT) credential required for the role.

Credentials & Documentation Required

Submission Required Before Start:

• Current professional license with primary source verification (within 30 days prior to assignment)
• Driver’s License or State ID (valid, not expired, matching address requirements)

• At least two professional references

Required Onboarding Credentials (must be completed/uploaded prior to or at start):

• Ascension Respiratory Therapy Contract Worker Onboarding Education Attestation (6 hours pre-hire training with signed attestation)
• Basic Life Support (BLS) – AHA certified
• Pediatric Advanced Life Support (PALS) – AHA certified
• Advanced Cardiovascular Life Support (ACLS) – AHA certified
• Neonatal Resuscitation Program (NRP) – AHA/American Academy of Pediatrics certified
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics) – completed within 1 year, minimum 80% score
• Ascension Biometric Consent
• Ascension Non-Nursing Contingent Worker Pre-Hire Modules/Attestation (Illinois specific)
• Ascension Badge Photo (forward-facing headshot, white or blue background, no filters)
• Health Statement or completed physical exam (updated annually)
• Fit Test (preferred before start, can be done onsite; annual update required)
• Hepatitis B vaccination series completion or signed declination
• Measles, Mumps, Rubella (MMR) – documented proof of two vaccines or positive titer (no declinations accepted)
• Varicella – documented proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
• Tdap – valid within 10 years (no declinations accepted)
• 2 Step PPD or IGRA (Quantiferon/Tspot) testing with annual TB questionnaire
• Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
• Ascension Criminal Background Check (including SSN trace, employment verification, OIG search, national sex offender registry)
• Ascension EPLS (Excluded Parties List System) check within 90 days of start
• Ascension Skills Checklist (clinical specialty, completed within 1 year, updated annually)
• Ascension Education Verification (within 90 days of start)
• Ascension Employment Verification (past 7 years, updated annually)
• Ascension Confirmation of Assignment (agency letterhead with start/end dates)

Optional / Conditional:

• Influenza Vaccine (recommended during flu season Oct 1 – Mar 31; declinations accepted with mask requirement)
• Positive PPD with negative chest X-ray and annual TB questionnaire (if applicable)
• Ascension Respiratory Therapy Contract Worker Form (completed onsite)
• Hiring Manager Evaluation (post-assignment)
• Ascension Name Change Documentation (if applicable)
