Job Description

• Outpatient Physical Therapist position
• Shared housing available (no guests or pets allowed)
• Must have own transportation

Required Credentials & Compliance for Onboarding

Professional Credentials

• MGB MVH Housing Agreement (if moving into shared housing)
• BLS Certification (AHA preferred; Red Cross accepted)
• License/Certificate Verification: Primary source verification of professional license required within 30 days of start date; MA Conditional Licenses not accepted
• MGB Criminal Background Check: Within 45 days prior to start date, including county and international searches if applicable
• MGB Massachusetts CORI: Criminal history check for MA within 30 days prior to start date
• MGB Resume: Past 7 years of work experience with city & state (required for submission)
• MGB References: Two recent references, at least one supervisor or above in skillset (required for submission)
• MGB Skills Checklist: Required for submission
• MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions
• MGB Education Verification: Verification of highest education level achieved
• MGB SSN Trace: Must be run with criminal check, covering last 7 years
• MGB National Sex Offender Check: Within 30 days of start and updated annually
• MGB Exclusion Checks: Within 30 days of start and annually; includes OIG, GSA, MassHealth & NH Medicaid exclusion lists

Personal Health History & Immunizations

• MMR: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test
• TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start date; updated if >6 month break
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start date; updated if >6 month break
• Respirator Medical Clearance (RMC) Attestation: Required if fit test is required for skill
• Influenza Vaccine: Required during flu season (Sept 15 - April 30); no exemptions allowed (optional outside flu season)
• Hepatitis B: Strongly preferred; record of 3 vaccines or proof of immunity or declination (optional)
• Tdap: Preferred vaccine within 10 years (optional)
• Positive PPD - Negative Chest X-Ray: Required if positive TB test (optional)
• MGB TB Symptom Survey: Required if positive PPD, updated annually (optional)
