Job Description

• Care for cardiac medical and surgical patients
• Experience with Impella and IABP preferred
• NxStage training preferred

Required Certifications & Licenses

• BLS (AHA)
• ACLS (AHA)
• Valid nursing license with primary source verification (Nursys verification within 30 days prior to start)
• Driver’s License/State ID (must be current and valid)
• NIH Certification (updated yearly or every 2 years depending on group)

Required Health & Immunization Documentation

• Proof of two MMR vaccines or positive IGG titer (no declinations accepted)
• Hepatitis B vaccination series completion or signed declination
• Varicella immunity proof or vaccination (no declinations accepted)
• Tdap vaccination (valid for 10 years, no declinations accepted)
• Influenza vaccine recommended during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement
• Positive IGRA or PPD results with negative chest X-ray within one year, plus annual TB questionnaire
• 2-Step PPD or IGRA within 90 days of start
• Health statement or completed physical exam prior to first assignment, updated annually
• Fit Test for respiratory protection (required for all travelers)
• 12 Panel Drug Screen within 30 days prior to start, updated annually

Required Professional Documentation & Verifications

• Resume/Profile submission
• At least two professional references prior to first assignment
• Criminal background check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and others (updated annually or after break in service > 90 days)
• Employment verification for the past seven years (updated annually or after break in service > 90 days)
• Education verification relevant to clinical work within 90 days of start
• Confirmation of assignment documentation upon initial assignment and each extension
• Nursys license verification within 30 days prior to start
• License/Certificate verification within 30 days prior to start
• Ascension Onboarding Requirements Education Attestation
• Ascension Nursing Contract Worker Onboarding

Required Exams & Competency Checklists

• Ascension Pharmacology Exam (minimum 80%, updated every 3 years)
• Ascension Specialty Exam (minimum 80%, updated every 3 years)
• Ascension Color Vision test (updated every 3 years)
• Ascension Core Competencies (HIPAA, Joint Commission, OSHA topics; minimum 80%, updated yearly)
• Ascension EKG Exam (minimum 80%, updated as per latest edition)
• Ascension Skills Checklist (clinical competency, updated annually)

Additional Notes

• Forward-facing headshot with white or blue background required for Ascension badge
• Some optional documents and checklists may be required by the facility/unit leader after onboarding
• Compliance with all credentialing and onboarding requirements is mandatory prior to assignment start
