Pharmacist Opportunity

Location: Mishawaka, Indiana
Facility: Saint Joseph Health System Mishawaka

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• Full-time, 72 hours per two-week rotation
• Work 7 consecutive days on, then 7 days off
• Evening/night shifts from 4:00 PM to 2:30 AM (last two night shifts extend to 3:30 AM to complete 72 hours)
• Rotations start 10/1/25 to 10/7/25 and continue every other week
• Pharmacists work every other weekend and are responsible for any weekend or holiday during their scheduled 7-day rotation

Requirements

• Indiana State Pharmacist License required
• Preferred experience with EPIC EMR system

Onboarding & Credentialing Requirements

Candidates must provide and maintain the following documentation, typically required before or at onboarding:

Certifications & Licenses

• BLS (American Heart Association) certification
• Current license/certificate verification dated within 30 days of start

Personal Health & Immunizations

• Fit Test
• Annual TB Questionnaire
• Trinity 10 Panel Drug Screen (9 Panel, no THC, completed within 30 days of start)
• COVID-19 Vaccine (including Sept 2024 booster, or approved declination/exemption)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (at least first shot prior to start)
• Influenza Vaccine (proof or declination for 10/1/2024 - 4/30/2025 season)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (per state guidelines)
• Varicella (proof of vaccination or IgG titer)
• Physical/Health Statement confirming fitness to work within the last year
• Trinity Color Vision test
• Trinity Latex Allergy Questionnaire
• Tdap vaccine (or declination if no pediatric interaction)

Professional & Compliance Forms

• Trinity Code of Conduct Form
• Trinity I9 form
• Confidentiality & System Security Agreement
• Integrity and Audit Services Form (signed shortly before start)
• Joint Commission CORE Exam (within the last year)
• Conflict of Interest Disclosure Form
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification)
• Education Verification (highest academic degree or diploma/GED)
• EPLS (Excluded Parties List System) check within 30 days of start
• GSA/OIG Exclusion check within 30 days of start
• NSO (National Sex Offender) check within 30 days of start
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days of start

This position is ideal for pharmacists licensed in Indiana who are comfortable with evening/night shifts and working a rotating schedule every other week, including weekends and holidays during their rotation. Experience with EPIC EMR is preferred.
