Job Description
- Evaluate and conduct medically prescribed physical therapy treatment programs.
- Perform initial and ongoing assessments of patient's condition.
- Establish, revise, and evaluate plans of care involving the patient and family.
- Perform therapy interventions using standard physical therapy techniques appropriate to patient condition.
- Oversee activities of physical therapy assistants, students, and support personnel.
Work Complexity:
- Requires critical thinking, decisive judgment, and ability to work with minimal supervision.
- Must be able to work in a stressful environment.
Personal Protective Equipment:
- Follow Standard Precautions and use PPE as required.

- Ascension Resume (submission required)
- Ascension Profile (submission required)
- Ascension 2 Professional References (submission required)
- Driver's License/State ID (submission required; must be current and address must match records)
- Ascension Badge Photo (onboarding required; forward-facing headshot with white or blue background, no alterations)
- Ascension Criminal Background Check Results (onboarding required; includes SSN trace, employment verification, OIG search, national sex offender check, updated annually)
- Ascension EPLS (onboarding required; updated within 90 days of start)
- Ascension Skills Checklist (onboarding required; clinical only, completed within one year of start, updated annually)
- Ascension Education Verification (onboarding required; must be completed within 90 days of start, updated if break in service > 90 days)
- Ascension Confirmation of Assignment (onboarding required; initial and updated upon extension)
- Ascension Employment Verification (onboarding required; past 7 years, updated annually)
Personal Health History (Required for Onboarding)
- MMR: Documented proof of two vaccines or positive IGG titer; no declinations accepted.
- Fit Test: Required for all travelers; preferred before start, can be completed onsite.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Recent testing required; TB questionnaire updated annually.
- Varicella: Documented proof of two vaccines, immunity, or history verified by healthcare provider; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually; includes 12 specified drugs.
Optional / Seasonal / Additional Information
- Influenza Vaccine: Optional; flu season October 1 through March 31. Exemptions accepted but require mask-wearing.
- Positive PPD with Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended if applicable.
- Ascension Exit Survey: Optional post-assignment survey.
- Hiring Manager Evaluation and Name Change Documentation: Optional.
This position is suitable for experienced Physical Therapists comfortable working independently in a fast-paced, stressful environment, with responsibility for patient care and oversight of support staff. Compliance with Ascension Health’s credentialing and health requirements is mandatory.
