Description

Job Opportunity: Interventional Radiology Technician

Location: Chicago, Illinois
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

Monday through Friday
On-call: 15 days per month
8-hour shifts starting at 06:45 AM

Required Experience

EPIC experience is a must

Professional Credentials

Driver's License, State ID, or Passport (required for onboarding)
Prime OIG clearance (onboarding required)
Prime SAM/EPLS verification (onboarding required)
Resume submission (required before submission)
Two professional references (required before submission)
Social Security Card (onboarding required)
Criminal Background Check (national, within one year of start)
Badge Photo (clear headshot, JPEG format)
Form I-9 (Employment eligibility verification)
Signed Job Description
Essentials Onboarding Packet completion
Onboarding Education Attestation (must be completed prior to clearance)

Personal Health History & Vaccinations

COVID-19 Vaccine: 2 doses plus booster or completed declination
MMR: Proof of 2 vaccines or positive titer showing immunity
Hepatitis B vaccination (declinations not accepted)
Influenza Vaccine or declination (required Nov 1 - Mar 31)
TB Test and Annual TB Questionnaire
Varicella: Vaccination or titer showing immunity
Tdap vaccination (declinations not accepted in some locations)
Fit Test: Annual respirator fit test must be completed prior to start (no onsite testing in Illinois)
Physical/Health Statement: Must state clear to work, completed annually or within 60 days of start

Testing & Competency

Prime Core Competencies Exam: Must score 80% or higher
Prime Skills Checklist (required before submission)
Prime Drug Screening: Minimum 10-panel test including THC, within 60 days of start

Additional Certifications

DCI/BVP (Workplace Violence Prevention training)

Optional

Name Verification Affidavit (if name discrepancies exist)
Certificate of Authenticity (COA)

This position requires thorough credentialing and compliance with health and safety standards prior to onboarding.
