Physical Therapist (PT) Job Opportunity
Location: Livonia, Michigan
Employment Type: Long-Term Assignment (LTA)
Contract Length: 10 weeks
Work Schedule & Environment
- Occasional weekends (1-2 days per month maximum)
- May work some holidays (at least one)
- Uses EPIC electronic health record system
- Coverage area includes Wayne and Washtenaw Counties
Required Credentials & Onboarding Requirements
Certifications & Licenses
- BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel. Must provide front and back images of the card.
- License/Certificate Verification: Must be dated within 30 days of start. Online verification with machine-generated date required.
Health & Drug Screening
- Fit Test: Required.
- 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days). Must include cocaine, amphetamines, opiates, 6-monoacetylmorphine, propoxyphene, PCP, barbiturates, benzodiazepines, methaqualone, and methadone. No THC allowed.
- Physical/Health Statement: Documentation from an exam within the last year confirming fitness to work.
Immunizations & Health History
- COVID-19 Vaccine: Proof of September 2024 booster, declination, or exemption (declination not accepted for CA or IL).
- Influenza Vaccine: Proof of vaccine, declination, or exemption for flu season 10/1/2024 - 4/30/2025 (declination not accepted for CA or IL).
- MMR: Proof of 2 vaccinations with dates or IgG titer lab report.
- Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
- Varicella: Proof of 1 vaccination with date or IgG titer lab report.
- Tdap: Current, non-expired vaccine required. Declinations accepted if no pediatric interaction.
Tuberculosis Screening
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required. Positive TB requires chest x-ray within last year for Michigan.
Professional & Compliance Requirements
- Trinity Code of Conduct Form: Signed form required.
- Confidentiality & Security Agreement: Signed form required.
- Integrity and Audit Services Form: Signed within days prior to start.
- Conflict of Interest Disclosure Form: Signed form required.
- Criminal Background Check: Federal, nationwide, county criminal check for last 7 years, including SSN verification and employment verification. Must be dated within 30 days of start.
- Education Verification: Highest academic degree or diploma/GED verified by third party screening.
- EPLS (Excluded Parties List System): Report dated within past 30 days required.
- Fingerprinting: State and department specific.
- GSA/OIG Exclusion Check: Report dated within past 30 days required.
- NSO (National Sex Offender) Check: Must be dated within 30 days of start, including statewide check for Michigan.
- Joint Commission CORE Exam: Within the last year.
- System Access Form: Must be completed and uploaded one week before start.
Skills & Competency
- Specialty Specific Competency Testing: Must be completed within the last year.
- Skills Checklist: Provided by agency for clinical contractors.
Identification
- Driver's License/State ID: Valid photo ID with all four corners visible plus insurance card photo required.
Submission Requirements
- Trinity Profile: Resume, skills checklist within a year, and 2 professional references required for submission.
This position requires thorough credentialing and compliance with health and professional standards prior to start.
