Position Details

Required Certifications and Licenses

• BLS (American Heart Association certification required for all direct patient care personnel)
• PALS (or Emergency Nurse Pediatric Course - ENPC)
• ACLS (required for RNs in ED, CCU, PACU, MICU, SICU)
• TNCC (Trauma Nursing Core Course)
• NIHSS (National Institutes of Health Stroke Scale)
• Valid nursing license with verification dated within 30 days of start

Health and Safety Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (completed within 30 days of start; must include specific substances; no THC)
• COVID-19 Vaccine (proof of September 2024 booster, declination or exemption accepted except in CA or IL)
• Influenza Vaccine (proof or declination/exemption; flu season dates apply)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (at least first vaccination prior to start; titers not accepted)
• Varicella (proof of vaccination or IgG titer)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Physical/Health Statement (exam within last year confirming fitness to work)
• Trinity Color Vision test

Professional and Compliance Requirements

• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed by agency)
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (completed and uploaded)
• Specialty Specific Competency Testing (copy of specialty exam within last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG (Office of Inspector General) exclusion check dated within 30 days
• Joint Commission CORE Exam (within last year)
• NSO (National Sex Offender) check dated within 30 days, including aliases and locations
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Tdap vaccine (current, non-expired; declinations accepted if no pediatric interaction)
• Child and Adult Dependent Abuse Training (required for Iowa)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days of start

• All certifications and licenses must be attached as scanned copies with expiration dates visible.
• Drug screen and background checks must be completed and cleared prior to start.
• Documentation must be uploaded into the designated credentialing system before onboarding.
• Compliance with all health and professional requirements is mandatory for assignment start.

This position is ideal for experienced Emergency Room RNs who meet the above credentialing and compliance requirements and are available for a 13-week long-term assignment in Des Moines, Iowa.
