- BLS (American Heart Association) - Required before onboarding
- Current nursing license with primary source verification within 30 days prior to start
- Nursys verification within 30 days prior to start
- Driver's License/State ID (must be valid and match address requirements)
- Ascension Profile (resume or professional profile)
- Ascension 2 Professional References (at least two professional references)
- Ascension Criminal Background Check (including SSN Trace, Employment Verification, OIG Search, National Sex Offender) within 30 days prior to start
- Ascension EPLS check within 90 days prior to start
- Ascension Education Verification within 90 days prior to start
- Ascension Employment Verification (7 years)
Exams & Testing (Must be completed prior to start and updated every 3 years unless otherwise noted)
- Ascension Pharmacology Exam (minimum 80%, max 2 attempts)
- Ascension Medical Surgical Exam (minimum 80%, max 2 attempts)
- Ascension Color Vision Test
- Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, max 2 attempts)
- Ascension EKG Exam (minimum 80%, max 2 attempts)
- Ascension Skills Checklist (clinical specialty skills; updated annually)
- Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
Onboarding Requirements
- Ascension Nursing Contract Worker Onboarding (completed onsite)
- Ascension Onboarding Requirements Education Attestation (6 hours pre-hire education)
- Ascension Badge Photo (professional headshot with white or blue background)
Personal Health History & Vaccinations
- MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
- Fit Test: Preferred prior to start, required annually
- Hepatitis B: Vaccination series completion or signed declination
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions require mask wearing
- Positive PPD with Negative Chest X-Ray and annual TB Questionnaire OR 2 Step PPD or IGRA test within 90 days prior to start
- Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
- Tdap: Valid within 10 years (no declinations accepted)
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually

- Patient ratio and unit specifics indicate a moderate acuity level with 23 beds on the Renal/Diabetic unit.
- Completion of various Ascension-specific exams and checklists is mandatory before starting the assignment.
- Background checks, education, and employment verifications are strictly required and must be current.
- Drug screening and health/vaccination requirements are comprehensive and must be up to date.
- Contract length is approximately 13 weeks.
This position is suitable for experienced Medical/Surgical RNs who meet the detailed credentialing and compliance requirements and are comfortable working in a renal/diabetic care setting with a patient ratio of 1:6-8.
