Job Summary

• Provide physical therapy services to patients in their homes with physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
• Patient age range: birth to over 100 years.
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate care.
• Maintain thorough and timely electronic documentation and interdisciplinary communication per agency policies and standards.
• Weekend rotations occur once every 6 weeks.
• Ability to travel to assigned cases is required.

Requirements

• Must hold a valid Physical Therapist professional license in Massachusetts.
• Minimum 1 year of home care experience required.

Onboarding & Credentialing Requirements

The following must be completed and verified prior to or at onboarding:

Licenses & Professional Credentials

• Massachusetts Physical Therapist License (Primary Source Verification; no conditional licenses accepted).
• Resume including past 7 years of work experience with city and state details.
• Two recent professional references (minimum one supervisor or above in skillset).
• Skills checklist relevant to the position.
• Criminal background check including:

  • Massachusetts CORI (state criminal history).
  • National sex offender registry check.
  • Exclusion checks (OIG, GSA, MassHealth, NH Medicaid).
  • Social Security Number trace for last 7 years of addresses.
• Education verification of highest degree achieved.
• Motor Vehicle Record (MVR) check (required for all home care positions).
• Confirmation of Assignment/POI form upon acceptance and for any extensions.

Health & Immunization Requirements

• Fit Test for N95 respirator within 365 days of start date, updated annually.
• Respirator Medical Clearance attestation (part of Fit Test documentation).
• TB screening: documentation of negative TB skin test or blood test within 3 months of start, updated if >6 month break.
• Health statement signed by MD/DO/NP/PA within 365 days of start, updated if >6 month break.
• MMR vaccination documentation or proof of immunity.
• Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions allowed.
• Hepatitis B vaccination strongly preferred (optional).
• Tdap vaccine within last 10 years preferred (optional).
• If positive TB test, documentation of negative chest x-ray required (optional).
• TB symptom survey if positive PPD (optional).

Additional Requirements

• MGB Badge Photo: recent color photo (no selfies, no filters, plain background, no eyeglasses).
• Nurse Aide Registry check within 30 days of start (required by MA DPH for all employees).

Notes

• No religious or medical exemptions allowed for fit testing or influenza vaccine.
• All documentation must be current and compliant with Massachusetts Department of Public Health regulations.
• Weekend rotations are infrequent (once every 6 weeks).
• Travel to patient homes is required as part of the role.
