Physical Therapist (PT) Travel Job - Waco, Texas

Job Description

• Perform initial and ongoing physical therapy assessments to establish, revise, and evaluate patient care plans.
• Utilize standard physical therapy techniques and skills appropriate to patient conditions.
• Oversee activities of physical therapy assistants and support personnel.
• Collaborate with physicians, nursing staff, and other healthcare professionals regarding patient progress and recommendations.

Required Credentials and Compliance

Personal Health History Requirements

• MMR Vaccine: Documented proof of two MMR vaccines or positive IGG titer. If negative, two doses of MMR vaccine four weeks apart required. No declinations accepted.
• Fit Test: Required for all travelers; preferred before start but can be completed onsite. Annual update required.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31). Exemptions accepted with mask requirement.
• TB Screening: Positive IGRA or PPD results, chest X-ray within one year showing no infection, and TB questionnaire completed. Annual TB questionnaire updates required.
• 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd PPD within 30 days of start, or negative IGRA within 90 days of start.
• Varicella: Proof of two vaccines, immunity evidence, or history verified by healthcare provider. If negative, two doses four weeks apart required. No declinations accepted.
• Tdap: Valid within 10 years. No declinations accepted.
• 12 Panel Drug Screen: Completed within 30 days prior to start and updated annually. Includes testing for 12 specified substances with MRO review if needed.
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.

Professional Requirements

• Driver's License/State ID: Must be current, match address on file, and comply with compact license rules.
• Resume/Profile Submission: Required.
• Professional References: At least two qualifying references prior to first assignment.
• Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check. Completed within 30 days of first assignment and updated annually.
• EPLS Check: Completed within 90 days of start and rerun if break in service > 90 days.
• Education Verification: Verification of relevant education within 90 days of start; rerun if break in service > 90 days.
• Employment Verification: Third-party verification for past 7 years, updated annually.
• Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
• Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.

Optional/Additional

• Post-assignment evaluations and surveys may be required.
• Legal documentation for name changes if applicable.
• Badge photo with specific background requirements.
