Job Opportunity: Therapy/Rehabilitation - Travel/Contract
Location: Wichita Falls, Texas
Contract Length: 13 weeks
Start Dates: 11/10, 11/17, or 12/8

Background Checks
- Background Check within 1 year of start (includes SS Trace, County searches for last 7 years, Nationwide, Sex Offender)
- EPLS/GSA/SAM check within 30 days of start
- HHS/OIG check within 30 days of start
Certifications (AHA ONLY unless otherwise noted)
- ACLS (Advanced Cardiac Life Support) – Instructor, Provider, or RQI accepted
- BLS (Basic Life Support) – Instructor, Provider, or RQI accepted
- PALS (Pediatric Advanced Life Support) – Instructor, Provider, or RQI accepted
- NRP (Neonatal Resuscitation Procedure) – Required as applicable
- CHEMO (Chemotherapy) – Required as applicable
- TNCC (Trauma Nursing Core Course) – Required as applicable
- EFM (Electronic Fetal Monitoring) – Required as applicable
- ENPC – Required as applicable
- NIH Stroke Certification – Required as applicable
- Tech Cert Verification (National) – Required within 30 days of start for Allied/Non-RN candidates
- Orientation Safety Manual Acknowledgement – Required, update annually
- State License Verification – Required within 30 days of start
- NURSYS Verification (RN, LPN/LVN, AP) – Required within 30 days of start
Competency Exams
- Medication Test/Pharmacology Competency Exam – Required as applicable
- Competency Exam – Required as applicable
Facility-Specific Requirements
- Flu Medical Exemption (if seeking exemption)
- Flu Religious Exemption (if seeking exemption)
- Parking Policy Acknowledgment – Must be completed prior to start
Health Requirements
- Physical Exam: Required within 1 year of start; must be completed within 72 hours of signing contract
- Tetanus/Diphtheria/Tdap: Vaccine required within 10 years of start
- TB Testing: Negative 2-step PPD, Q Gold, or TSpot within 1 year of start; chest X-ray within 5 years if positive PPD
- TB Questionnaire: Required for all candidates
- Rubeola (Measles): 2 vaccines or positive titer required; candidates born before 1957 are exempt
- Mumps: 2 vaccines or positive titer required; candidates born before 1957 are exempt
- Rubella: 2 vaccines or positive titer required; candidates born before 1957 are exempt
- Hepatitis B: Positive titer, series (3), or declination required
- Varicella: 2 vaccines, positive titer, or history of chickenpox required
- Drug Testing: Negative 9-panel drug screen within 1 year of start; must have plan to complete within 72 hours of signing contract
- Flu Seasonal Vaccination: Required during flu season (10/1-5/31); medical/religious exemptions allowed subject to review
- COVID-19 Vaccine: Preferred but not required
