Job Details

• Specialty: Registered Nurse in Cardiovascular Operating Room (RN CVOR)
• Holiday rotation as needed
• On-call requirement: 10-12 days per month

Required Credentials & Certifications

Licenses & Certifications (Must be current and verified within 30 days of start)

• RN License (state verification required)
• BLS (American Heart Association certification required for all direct patient care personnel)
• ACLS (required for RNs in critical care areas)
• NIHSS
• TNCC (optional)

Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen (completed within 30 days of start; no THC)
• COVID-19 Vaccine (including booster by Sept 2025, or approved declination/exemption)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B (must have started vaccination series prior to start)
• Influenza Vaccine (proof or declination for current flu season)
• 2 Step PPD OR IGRA (TB test; chest x-ray if positive)
• Varicella (proof of vaccination or IgG titer)
• Tdap (current, non-expired; declination accepted if no pediatric interaction)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Color Vision Test

• Trinity Code of Conduct Form (signed)
• Trinity I9 Form (completed by agency)
• Confidentiality & System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28 completed)
• Specialty Specific Competency Testing (completed within last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Excluded Parties List System) check within 30 days
• GSA/OIG Exclusion Check within 30 days
• Joint Commission CORE Exam (within last year)
• NSO (National Sex Offender) check within 30 days
• System Access Form (completed and uploaded one week before start)
