Physical Therapist (PT)

Location: Cheraw, South Carolina
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Description

• Maintain a professional image and excellent customer relations with patients, visitors, physicians, and co-workers.
• Collaborate with staff to develop professional skills, new programs, and patient treatment plans.
• Perform treatments as ordered in a safe and timely manner, involving staff and family when appropriate.
• Complete medical documentation according to APTA standards of care.
• Participate in services, meetings, and educational sessions.
• Assist in adjusting schedules to accommodate changes in caseload.
• Ensure a clean, orderly, and safe work environment.
• Evaluate patients referred for physical therapy using specialized testing and assessment of strength, range of motion, coordination, sensation, ambulation, transfers, bed mobility, and rehabilitation potential.
• Develop individualized physical therapy plans of care with short- and long-term goals based on evaluation data.
• Supervise treatment team operations and regularly meet with PTAs to discuss patients and set goals.
• Provide evidence-based care and seek out research to support interventions.
• Maintain patient confidentiality and adhere to all facility and corporate policies and procedures.

Required Credentials & Compliance

• BLS Certification (AHA only)
• Physical Therapy License/Certificate (Primary source verification required, state-specific registry)
• Immunizations:

  • MMR (Proof of 2 immunizations or titer with lab report; no declinations accepted)
  • Hepatitis B (Proof of 3 vaccinations or titer with lab report; no declinations accepted)
  • Varicella (Proof of 2 immunizations or titer with lab report; no declinations accepted)
• Tuberculosis Testing:

  • 2 Step PPD OR IGRA (Quantiferon/Tspot) with specific timing requirements
• Driver’s License/State ID (valid and up to date)
• Social Security Card (copy required)
• Criminal Background Check: County, Statewide, National, OIG, EPLS, OFAC, NSOP, SS Trace covering last 7 years, done within 60 days of start and updated annually
• Employment Verification: 5 years
• Drug Screen: 10 panel drug screen completed within 30 days of start (rapid screen not accepted)
• Skills Checklist: Clinical skills checklist completed prior to start and updated annually
• Assignment Confirmation: Standard form required

Optional / Seasonal Requirements

• Fit Test (required for travelers with specific mask make/model within one year of start)
• Influenza Vaccine (required September through April; medical/religious exemptions considered case-by-case)
• Annual Negative PPD, Annual TB Questionnaire, Positive PPD with Negative Chest X-Ray (if applicable)
