Physical Therapist (PT) - Florence, South Carolina

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Description

• Maintain a professional image and provide excellent customer relations with patients, visitors, physicians, and co-workers.
• Collaborate with staff to develop professional skills, new programs, and patient treatment plans.
• Perform treatments as ordered in a safe and timely manner, involving staff and family members when appropriate.
• Document medical records in accordance with APTA standards of care.
• Participate in services, meetings, and educational sessions.
• Assist in adjusting schedules to accommodate changes in caseload.
• Ensure a clean, safe, and orderly work environment.
• Evaluate patients referred for physical therapy using specialized testing procedures and data from medical records.
• Develop individualized physical therapy plans of care based on evaluations, targeting short- and long-term goals.
• Supervise treatment team operations and regularly meet with PTAs to discuss patient progress and goals.
• Provide evidence-based care and seek out research to support interventions.
• Maintain confidentiality of patient records and facility operations.
• Adhere to all facility and corporate policies and procedures.

Required Credentials & Health Requirements

• Certifications:

  • BLS (AHA only) - Required for onboarding

• Licenses:

  • State License/Certificate Verification (Primary source verification required) - Required for onboarding

• Professional Documents:

  • Valid Driver's License or State ID - Required for onboarding
  • Social Security Card - Required for onboarding
  • Criminal Background Check (County, Statewide, National, OIG, EPLS, OFAC, NSOP, SS Trace; within 60 days of start, covering last 7 years) - Required for onboarding
  • Employment Verification (5 years) - Required for onboarding
  • Confirmation of Assignment (facility standard form) - Required for onboarding
  • Resume/Profile (references preferred but not required) - Required for submission
  • Skills Checklist (clinical, completed prior to start, updated annually) - Required for submission

• Personal Health History:

  • MMR (Proof of 3 immunizations or titer with lab printout; no declinations accepted) - Required for onboarding
  • Hepatitis B (Proof of 2 vaccinations or titer with lab printout; no declinations accepted) - Required for onboarding
  • Varicella (Proof of 2 immunizations or titer with lab printout; no declinations accepted) - Required for onboarding
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) (1st within 1 year, 2nd within 30 days of start; negative Q-Gold or Tspot acceptable if within 30 days) - Required for onboarding

• Optional/Seasonal:

  • COVID-19 Vaccine (Fully vaccinated card preferred; medical/religious exemptions considered case-by-case)
  • Fit Test (required for travelers with make/model, completed within 1 year of start)
  • Influenza Vaccine (required October - April; exemptions considered case-by-case)
  • Covid Test
  • Annual Negative PPD (if 2 step PPD not done within past year)
  • Annual TB Questionnaire
  • Positive PPD with Negative Chest X-Ray (X-ray within 90 days of start)

• Drug Screening:

  • 10 panel drug screen completed within 30 days of start - Required for onboarding
