MRI Technician - Davenport, Iowa (LTA Contract)

Assignment Description

• Work at an imaging center within a hospital system.
• 16-week assignment with a full-time schedule.

Required Credentials & Compliance for Onboarding

Certifications & Licenses

• BLS (American Heart Association certification required for all direct patient care personnel; must provide front and back card images)
• ARRT (American Registry of Radiologic Technologists)
• ARMRIT (American Registry of Magnetic Resonance Imaging Technologists)
• License/Certificate Verification (dated within 30 days of start; state verification required)

Health & Immunizations

• Fit Test (Iowa - 1860 mask only)
• Trinity 10 Panel Drug Screen (9 or 10 panel drug screen completed within 30 days of start, no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, declination or exemption accepted except for CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B (must have started vaccination series prior to start; titers not accepted)
• Influenza Vaccine (proof or declination for 2024-2025 or 2025-2026 flu season; declination not accepted for CA or IL)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (within 1 year for some states, 5 years for others)
• Varicella (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap (current vaccine required; declination accepted if no pediatric interaction)

Professional & Compliance Forms

• Trinity Code of Conduct Form (signed)
• Trinity I9 (completed by agency)
• Confidentiality & System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non Employee Upload Form (pages 1-3 and 28 completed)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification; dated within 30 days)
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) check (dated within 30 days)
• GSA/OIG Exclusion Check (dated within 30 days)
• Joint Commission CORE Exam (within 1 year)
• NSO (National Sex Offender) check (dated within 30 days; includes aliases and locations)
• System Access Form (completed and uploaded one week before start)
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days)

Testing & Competency

• Specialty Specific Competency Testing (copy of specialty exam within last year)
• Skills Checklist (provided by agency for clinical contractors)
