Job Opportunity: RN Cath Lab - Waterloo, Iowa

Position Details

• Specialty: RN Cath Lab

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Schedule: Every third weekend; call weekends and weekdays same as full-time staff; couple of days a week; holiday rotation

Certifications & Licenses

• BLS (American Heart Association certification required; front and back of card needed)
• ACLS
• License/Certificate Verification (must be dated within 30 days of start; online verification with machine-generated date required)

Personal Health History & Testing

• Fit Test
• NIHSS Certification
• Trinity 10 Panel Drug Screen (10-panel drug screen completed within 30 days of start; no THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, declination or exemption accepted except for CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B (must have started vaccination series prior to start; titers not accepted)
• Influenza Vaccine (proof or declination; declination not accepted for CA or IL; valid for flu season 10/1/2024-4/30/2025)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB (timing varies by state)
• Varicella (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap Vaccine (current, non-expired; declinations accepted if no pediatric interaction)

Professional & Compliance Forms

• Trinity Code of Conduct Form (signed)
• Trinity I9 (completed by agency)
• Confidentiality and System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28 completed and uploaded)
• Specialty Specific Competency Testing (copy of specialty exam within last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (federal, nationwide, county for last 7 years; SSN verification; dated within 30 days)
• Education Verification (highest academic degree or diploma/GED verified)
• EPLS (Exclusions) Report (dated within past 30 days)
• GSA/OIG Exclusion Check (dated within past 30 days)
• Joint Commission CORE Exam (within last year)
• NSO (National Sex Offender) Check (dated within 30 days; includes aliases and locations)
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (required for Iowa assignments)
• Iowa Sing Report (Iowa-specific criminal history, sex offender, dependent adult & child abuse registry check within 30 days prior to start)
• OFAC Sanctions Check (completed within 30 days of start)
