Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Schedule: Every third weekend

Required Credentials & Certifications

Licenses & Certifications (Must be current and verified within 30 days of start)

• Registered Nurse License (state verification required)
• BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel.
• ACLS (Advanced Cardiovascular Life Support) – Required for RNs in CCU, ED, PACU, MICU, SICU.
• NIHSS (National Institutes of Health Stroke Scale)
• TNCC (Trauma Nursing Core Course) – Optional but preferred.

Health & Immunizations

• Fit Test
• Trinity 10 Panel Drug Screen (no THC) – Completed within 30 days of start, preferred within 14 days.
• COVID-19 Vaccine – Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL).
• Influenza Vaccine – Proof or declination/exemption for current flu season (declination not accepted in CA or IL).
• MMR (Measles, Mumps, Rubella) – Proof of 2 vaccinations or IgG titer.
• Hepatitis B – Must have started vaccination series prior to start; titers not accepted.
• Varicella – Proof of vaccination or IgG titer.
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – TB screening with chest x-ray if positive (per regional requirements).
• Tdap – Current, non-expired vaccine required; declination accepted if no pediatric interaction.
• Physical/Health Statement – Documentation from exam within last year confirming fitness to work.
• Trinity Color Vision

Professional & Compliance Requirements

• Trinity Profile – Resume, skills checklist (within 1 year), and 2 professional references required for submission.
• Trinity Code of Conduct Form – Signed and on file.
• Trinity I9 – Completed by agency.
• Confidentiality & Security Agreement – Signed form required.
• Integrity and Audit Services Form – Signed shortly before start.
• Non-Employee Upload Form – Completed and uploaded.
• Specialty Specific Competency Testing – Specialty exam completed within the last year.
• Conflict of Interest Disclosure Form – Signed and on file.
• Criminal Background Check – Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start.
• Education Verification – Highest academic degree or diploma verified by third party screening.
• EPLS (Excluded Parties List System) Check – Dated within 30 days of start.
• GSA/OIG Exclusion Check – Dated within 30 days of start.
• Joint Commission CORE Exam – Completed within the last year.
• NSO (National Sex Offender) Check – Dated within 30 days of start, including all aliases and locations.
• System Access Form – Completed and uploaded one week before start.
• Skills Checklist – Provided by agency for clinical contractors.
• Child and Adult Dependent Abuse Training – Required for Iowa assignments.
• Iowa Sing Report – Criminal history, sex offender, dependent adult & child abuse registry check required within 30 days prior to start.
• OFAC (Office of Foreign Assets Control) Sanctions Check – Completed within 30 days of start.

This is a 13-week long-term assignment for an RN with Medical/Surgical and Telemetry experience in Mason City, Iowa. The position requires weekend availability every third weekend.
