Job Opportunity: Cardiovascular Technologist

Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• 40 hours per week
• Day shift with various start times
• On-call: 2 days per week, every 3rd week

Required Certification

• BLS (American Heart Association)

Required Credentials and Documentation for Onboarding

Candidates must provide the following, all to be current and compliant with specified timelines:

Certifications & Licenses

• BLS certification (front and back images)
• License/Certificate verification dated within 30 days of start

Health & Immunizations

• Fit Test
• 10 Panel Drug Screen (completed within 30 days of start, no THC)
• COVID-19 Vaccine (including Sept 2025 booster, or approved declination/exemption)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (at least first shot prior to start)
• Influenza Vaccine (proof or declination for 10/1/2024 - 4/30/2025)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Varicella (proof of 1 vaccination or IgG titer)
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap vaccine (current and non-expired; declination accepted if no pediatric interaction)

Professional & Background Checks

• Trinity Profile (resume, skills checklist within a year, 2 professional references)
• Criminal Background Check (Federal, Nationwide, County for last 7 years, including SSN verification, dated within 30 days)
• Education Verification (highest academic degree or diploma/GED)
• EPLS (Excluded Parties List System) report dated within 30 days
• GSA/OIG exclusion check dated within 30 days
• NSO (National Sex Offender) check dated within 30 days
• Iowa-specific Child and Adult Dependent Abuse Training and registry checks (for Iowa assignments)
• OFAC Sanctions check within 30 days
• Trinity Code of Conduct Form (signed)
• Trinity Confidentiality of Patient & Proprietary Information and System Security Agreement (signed)
• Trinity Integrity and Audit Services Form (signed shortly before start)
• Trinity Non Employee Upload Form (pages 1-3 and 28 completed and uploaded)
• Trinity Conflict of Interest Disclosure Form (signed)
• Trinity Joint Commission CORE Exam (within last year)
• Trinity System Access Form (completed and uploaded one week before start)
• Trinity Specialty Specific Competency Testing (specialty exam within last year)
• Trinity Skills Checklist (provided by agency for clinical contractors)
