Physical Therapist Outpatient - Oak Bluffs, Massachusetts

Employment Type: Long-Term Assignment (LTA)

Schedule: Monday - Friday, 9 AM - 5 PM
Location: Outpatient setting

Job Requirements and Qualifications

• Must have own transportation
• Candidate must have received the influenza vaccine (required during flu season: Sept 15 - April 30)
• Shared housing available (no guests or pets allowed) — housing agreement required if using shared housing

Required Credentials and Documentation (Must be completed prior to or at onboarding)

Professional Credentials

• Resume including past 7 years of work experience (with city & state)
• Two recent references (minimum one supervisor or above in relevant skillset)
• Skills checklist specific to the role
• Confirmation of assignment form (for initial assignment and extensions)
• Criminal background check (within 45 days prior to start, updated if >6 month break)
• Massachusetts CORI background check (within 30 days prior to start, updated if >6 month break)
• Education verification of highest level achieved
• Social Security Number trace (addresses/aliases for past 7 years)
• National sex offender registry check (within 30 days of start, updated annually)
• Exclusion checks (OIG, GSA, MassHealth, NH Medicaid) within 30 days of start and annually

Licenses and Certifications

• Professional license verification (primary source verification within 30 days of start, updated upon expiration)
• Basic Life Support (BLS) certification (AHA, Red Cross, CHSF, or Military accepted)

Personal Health History and Immunizations

• Influenza vaccine (required during flu season, no exemptions)
• Measles, Mumps, Rubella (MMR) vaccination or proof of immunity
• Tuberculosis (TB) test documentation within 3 months of start (skin test or blood test)
• Health statement signed by MD/DO/NP/PA within 365 days of start
• Respirator Medical Clearance attestation (if fit test required)

Optional / Preferred

• Hepatitis B vaccination or proof of immunity (strongly preferred)
• Tdap vaccine within last 10 years (preferred)
• Documentation of chest x-ray if positive TB test

Additional Notes

• MA Conditional Licenses are not accepted
• No religious or medical exemptions allowed for influenza vaccine
• Background checks include international searches if applicable
• Housing agreement required only if moving into shared housing
