Required Credentials & Compliance for Onboarding

Professional Credentials

• Resume: Include past 7 years of work experience with city & state details. (Required for submission)
• References: Two recent references, at least one supervisor or above in the skillset. (Required for submission)
• Skills Checklist: Must be completed. (Required for submission)
• Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
• Criminal Background Check: Within 45 days prior to start, including county and international searches if applicable.
• Massachusetts CORI Check: Completed within 30 days prior to start.
• Education Verification: Verification of highest education level achieved.
• SSN Trace: Must be run with criminal check covering last 7 years.
• National Sex Offender Check: Within 30 days of start and updated annually.
• Exclusion Checks: OIG, GSA, MassHealth & NH Medicaid exclusion lists checked within 30 days of start and annually.

Licenses & Certifications

• License/Certificate Verification: Primary source verification required within 30 days of start. MA Conditional Licenses not accepted.
• BLS Certification: AHA, Red Cross, CHSF, or Military accepted. Preferred AHA for certain hospitals.

Personal Health History & Vaccinations

• Flu Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
• MMR: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
• TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start.
• Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start.
• Respirator Medical Clearance Attestation: Required if fit test is needed for the role.

Optional but Preferred

• Hepatitis B: Record of 3 vaccines or proof of immunity or declination.
• Tdap: Vaccine within last 10 years, updated upon expiration.
• Positive PPD - Negative Chest X-Ray: If applicable.
• TB Symptom Survey: If positive PPD, to be completed prior to start and updated annually.

Additional Notes

• Candidate must have flu vaccine.
• Shared housing requires signing the MGB MVH Housing Agreement if utilized.
• All documentation must be current and updated as required, especially if there is a break in service longer than 6 months.
