Job Details

• Start Date: January 5 (Non-negotiable)
• Shift Length: 36 or 48 hours per week
• Floating: May be required to float between units or facilities within a 35-mile radius based on staffing needs, patient census, and acuity. Notification will be given before the scheduled shift if floating is required.

Required Credentials & Compliance

Licenses & Certifications

• Current nursing license with primary source verification within 30 days prior to start
• Nursys verification within 30 days prior to start
• BLS certification (American Heart Association)
• NIH Certification (Group A updated yearly; Groups B-F every 2 years)

Health & Immunizations

• Influenza vaccine or mask requirement during flu season (Oct 1 - Mar 31)
• MMR: Proof of two vaccines or positive titer; no declinations accepted
• Varicella: Proof of two vaccines, immunity, or history; no declinations accepted
• Tdap: Valid within 10 years; no declinations accepted
• Hepatitis B: Vaccination series completion or signed declination
• Fit Test: Required for all travelers, preferred before start; updated annually
• 2-Step PPD or IGRA testing with annual TB questionnaire
• Positive PPD requires negative chest X-ray within 1 year and TB questionnaire
• Health statement or physical exam prior to first assignment, updated annually

Testing & Exams

• Ascension Medical Surgical Exam: Minimum 80% score, max 2 attempts, updated every 3 years
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, updated yearly
• Ascension EKG Exam: Minimum 80% score, max 2 attempts, updated as per latest edition
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation
• Ascension Skills Checklist: Clinical skills, completed within 1 year of start, updated annually
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually; includes 12 specified drugs

Onboarding & Documentation

• Ascension Resume/Profile
• Two professional references prior to first assignment
• Criminal background check including SSN trace, employment verification, OIG search, national sex offender registry, updated annually or after break in service > 90 days
• EPLS check within 90 days of start, rerun if break in service > 90 days
• Education verification relevant to clinical work within 90 days of start, rerun if break in service > 90 days
• Employment verification for past 7 years, updated annually or after break in service > 90 days
• Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
• Ascension Nursing Contract Worker Onboarding (done onsite)
• Ascension Nova StatStrip pre-hire modules for non-allied positions
• Ascension Confirmation of Assignment on agency letterhead with accurate start/end dates
• Driver’s License/State ID (valid, matching address requirements)
• Forward-facing badge photo with white or blue background, no alterations

Optional / Additional Information
