Required Credentials & Compliance for Submission

• Resume/Profile: Must submit a current professional resume or profile.
• Nursys License Verification: Verification within 30 days prior to start. Complete FACIS Questionnaire if any disciplinary actions exist.
• License/Certificate Verification: Current license and primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
• Driver's License/State ID: Must be valid, not expired, and match address on file. Front and back copies required.

Required Credentials & Compliance for Onboarding

• BLS Certification: Must be American Heart Association (AHA) certified.
• Ascension Medical Surgical Exam: Must be completed prior to start; minimum 80% score; only 2 attempts allowed; updated every 3 years.
• Ascension Color Vision Test: Completed prior to start and updated every 3 years.
• Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% score; only 2 attempts allowed.
• NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
• Ascension EKG Exam: Must be completed with at least 80% passing score; only 2 attempts allowed; updated as per latest edition.
• Ascension Onboarding Education Attestation: Completion of 6-hour pre-hire training with attestation upload required.
• Ascension Nursing Contract Worker Onboarding: Completed onsite.
• Ascension Nova StatStrip: Pre-hire modules required for non-allied positions, completed prior to clearance.
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
• MMR Vaccination: Documented proof of two MMR vaccines or positive IGG titer required; no declinations accepted. If negative titer, two doses of MMR vaccine four weeks apart required.
• Fit Test: Required for all travelers; preferred prior to start but can be completed onsite; updated annually.
• Hepatitis B: Documentation of vaccination series completion or signed declination required.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted but mask wearing required at all times.
• Health Statement: Statement of good health or completed physical exam prior to first assignment; updated annually.
• 2 Step PPD or IGRA (Quantiferon/Tspot): 1st PPD within 1 year, 2nd PPD within 30 days of start, or negative IGRA within 90 days prior to start. TB questionnaire updated annually. Positive TB requires chest X-ray within last year.
• Varicella: Proof of two vaccines, serological immunity, or verified history required; no declinations accepted. If negative, two doses of vaccine four weeks apart required.
• Tdap: Valid within 10 years; no declinations accepted.
• Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; completed within 30 days of first assignment; updated annually or if break in service > 90 days.
• Ascension EPLS Check: Completed within 90 days of start; rerun if break in service > 90 days.
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start; updated annually for specialty.
• Ascension 12 Panel Drug Screen: Completed within 30 days prior to start; updated annually; retesting required if break in service > 90 days. Includes testing for 12 specified drugs with MRO review if applicable.
• Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; rerun if break in service > 90 days. Official transcripts required if verification fails.
• Ascension Confirmation of Assignment: Provided upon initial assignment and updated with each extension; must be on agency letterhead with accurate start/end dates.
• Ascension Employment Verification: Third-party verification for past 7 years; updated annually or if break in service > 90 days.
• Ascension Badge Photo: Forward-facing headshot with white or blue background; no alterations or filters.

Optional / Additional Information

• Influenza Vaccine Exemption/Declination: Accepted during flu season but requires mask wearing at all times.
• Positive PPD with Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended to provide positive IGRA or PPD results, chest X-ray within a year, and TB questionnaire.
• Ascension Unit Specific Checklist: Optional for onboarding; must be uploaded by unit leader within a week.
• Hiring Manager Evaluation: Optional post-assignment evaluation.
• Ascension Name Change Document: Optional legal documentation of name change.

This position requires strict adherence to credentialing, testing, and vaccination requirements prior to and during the assignment. The start date is fixed, and the contract length is approximately 13 weeks.
