Job Opportunity: RN Medical/Surgical

Location: Nashville, Tennessee
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 36 or 48 hours per week
Start Date: January 12 (Non-negotiable)


Required Credentials & Onboarding Requirements

Licenses & Certifications

  • Nursys Verification: Must be completed within 30 days prior to start. If any disciplinary actions, complete FACIS Questionnaire.
  • License/Certificate Verification: Current license and primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
  • BLS (AHA): Required prior to onboarding.
  • NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
  • Driver's License/State ID: Must be current, match address on file, and show front/back.

Testing & Exams

  • Ascension Medical Surgical Exam: Must be completed prior to start; minimum 80% score; only 2 attempts allowed; updated every 3 years.
  • Ascension EKG Exam: New edition as of Dec 1, 2023; minimum 80% score; only 2 attempts allowed.
  • Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% score; only 2 attempts allowed.
  • Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required prior to start.
  • Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.

Health & Immunizations

  • MMR: Proof of two vaccines or positive IGG titer required; no declinations accepted. If negative titer, two doses of vaccine four weeks apart required.
  • Varicella: Proof of two vaccines, serological immunity, or verified history required; no declinations accepted. If negative, two doses of vaccine four weeks apart required.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Influenza Vaccine: Required for flu season (Oct 1 - Mar 31); exemptions accepted but mask required at all times.
  • Hepatitis B: Documentation of vaccination series completion or signed declination required.
  • Fit Test: Required for all travelers; preferred prior to start but can be completed onsite (except PAPR). Updated annually.
  • 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd PPD within 30 days of start; or negative IGRA within 90 days prior to start. TB questionnaire updated annually. Positive TB requires chest X-ray within 1 year and questionnaire.
  • Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended; must provide positive IGRA or PPD results, chest X-ray, and TB questionnaire.
  • Health Statement: Statement of good health or completed physical exam prior to first assignment; updated annually.
  • Ascension 12 Panel Drug Screen: Within 30 days prior to start; updated annually; includes testing for 12 specified drugs. Retesting required if break in service > 90 days.

Professional Documentation

  • Resume/Profile: Required for submission.
  • Two Professional References: Required prior to first assignment.
  • Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; completed within 30 days prior to start and updated annually or after break in service > 90 days.
  • EPLS Check: Completed within 90 days prior to start; rerun if break in service > 90 days.
  • Education Verification: Must be completed within 90 days prior to start; rerun if break in service > 90 days. Official transcripts required if verification fails.
  • Employment Verification: For past 7 years; updated annually or after break in service > 90 days.
  • Confirmation of Assignment: Provided upon initial assignment and updated with each extension.

Onboarding & Training

  • Ascension Nursing Contract Worker Onboarding: Completed onsite.
  • Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire training with attestation required.
  • Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
  • Ascension Badge Photo: Forward-facing headshot with white or blue background; no alterations or filters.

Optional

  • Ascension Unit Specific Checklist: Optional for onboarding; must be uploaded by unit leader within one week.
  • Hiring Manager Evaluation: Post-assignment evaluation (optional).
  • Ascension Name Change Document: Legal documentation of name change (optional).

This position requires strict adherence to credentialing, testing, and health requirements prior to and during the assignment. The start date is fixed, and the contract length is approximately 13 weeks.
