Key Requirements and Credentials

Licenses & Certifications

• Current Nursing License: Must have primary source verification within 30 days prior to start.
• Nursys Verification: Required within 30 days prior to start.
• BLS Certification: Must be American Heart Association (AHA) certified.
• Driver’s License/State ID: Must be current and valid, matching address requirements.

Health & Immunizations

• MMR: Proof of two vaccines or positive titer required; no declinations accepted.
• Varicella: Proof of immunity or two doses of vaccine required; no declinations accepted.
• Tdap: Valid within 10 years; no declinations accepted.
• Hepatitis B: Documentation of vaccination series completion or signed declination.
• Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement.
• Fit Test: Required for all travelers; preferred before start but can be done onsite.
• Positive PPD: Negative chest X-ray and annual TB questionnaire required if applicable.
• 2 Step PPD or IGRA: Required with specific timing before start; annual updates required.
• Health Statement: Statement of good health or physical exam prior to assignment, updated annually.
• 12 Panel Drug Screen: Within 30 days prior to start and annually thereafter.

Onboarding & Training

• Ascension Core Competencies: HIPAA, Joint Commission, OSHA topics; minimum 80% score, two attempts allowed.
• Ascension EKG Exam: Passing score of 80% required; two attempts allowed.
• Ascension Nursing Contract Worker Onboarding: Completed onsite.
• Ascension Onboarding Education Attestation: Completion of 6-hour pre-hire training.
• Ascension Nova StatStrip: Pre-hire modules required.
• Ascension Cerner Test-Out: Cerner test results and training checklist required.
• Ascension Skills Checklist: Clinical skills checklist completed within one year and updated annually.

Professional Documentation

• Resume/Profile: Required for submission.
• Two Professional References: Required prior to first assignment.
• Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually.
• Employment Verification: Past seven years required; updated annually.
• Education Verification: Relevant clinical education verified within 90 days of start.
• Confirmation of Assignment: Provided upon initial assignment and updates for extensions.

Additional Notes

• Influenza Vaccine Exemption: Accepted with mask-wearing requirement during flu season.
• Badge Photo: Forward-facing headshot with white or blue background required.
• Optional: Unit-specific checklist and hiring manager evaluation may be requested.

This position requires thorough credentialing and compliance with Ascension Health’s onboarding and health standards. The assignment is a fixed 13-week contract starting mid-December with options for 36 or 48 hours per week.
