Job Opportunity: Occupational Therapist (Travel/Contract)

Location: Lafayette, Indiana
Specialty: Therapy/Rehabilitation
Contract Length: 13 weeks


Work Schedule

  • Monday through Friday
  • One weekend day per month

Requirements

  • Minimum 1 year experience as an Occupational Therapist in an acute hospital setting
  • Indiana state OT license
  • NBCOT certification
  • BLS certification (AHA only, RQI acceptable)
  • Manual Therapy and Lymphedema certifications/licenses preferred
  • Cerner charting experience preferred

Job Description

  • Join an inpatient acute care rehab team of 19 professionals
  • Treat patients across all acuity levels: ICU to progressive care units
  • Patient diagnoses include CABG and other cardiac procedures, stroke, TBI, joint replacement, fracture repairs, respiratory illnesses, dementia, MS, Parkinson’s, ambulation issues, etc.
  • Responsibilities include evaluating patients, developing and following plans of care (POC) that may involve ADL training, therapeutic exercises, and activities to improve function
  • Daily assessment of discharge disposition appropriateness is expected

  • Scrub color: Royal Blue
  • Shoes: Black
  • Charting system: Cerner

Important Notes

  • Effective 11/30/2024, Indiana RNs will not be allowed to start new contracts at this facility except for Perioperative/OR, Allied, and Ambulatory RN travelers (including ENDO, IR RN, PACU, Cath Lab RN, SDS)

Required Certifications and Documents

  • Licenses & Certifications:
    • Indiana state license verification (within 30 days before start)
    • BLS (AHA only, RQI acceptable)
    • ACLS, PALS, NRP, CHEMO, TNCC, EFM, NIH Stroke Certification as applicable
  • Background Checks:
    • Comprehensive background check within 90 days of start (7-year history, SSN trace, county/state/national checks, sex offender registry, FACIS Level III)
    • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
  • Education Verification:
    • Official transcript or diploma required (may be pending at start)
    • Education verification by third party
  • Health Documents:
    • Physical exam within 12 months of start
    • Fit mask test within 12 months (approved respirators only)
    • Negative 10-panel drug screen within 12 months
    • Immunizations and titers:
      • Tetanus/Diphtheria/Tdap (preferred)
      • TB test (PPD, QuantiFERON, or T-SPOT) within 1 year
      • Rubeola (Measles), Mumps, Rubella, Varicella: positive titer or 2 vaccines required; no declinations accepted
      • Hepatitis B series, titer, or signed declination required
      • Seasonal flu vaccination or approved medical/religious exemption required for flu season (9/30 - 3/31)
  • Competency Exams:
    • Medication/Pharmacology competency exam
    • Unit competency exam
    • Color blind testing (Ishihara or equivalent)
  • Facility Specific Documents:
    • Compliance checklist, confidentiality agreement, policy information, orientation packets, and other facility-required forms must be completed as a priority

References and Employment History

  • Minimum of 2 references from recent employment within 2 years
  • References must be from charge, manager, supervisor, or higher (team lead acceptable for Allied positions)
  • Current employment application with full work history and explanations for gaps of 30+ days
  • Skills checklist completed within 12 months and updated annually
