Job Opportunity: Home Health Physical Therapist (PT)

Job Description

• Provide home health physical therapy services covering 13 counties.
• Weekly rotation includes travel to the furthest areas; fewer patients scheduled on high mileage/travel days.
• Shift options:

  • 5 days a week, 8 hours per day (5x8)
  • 4 days a week, 10 hours per day (4x10)
  • Start time: 8:00 AM
• Call requirement: 1 Sunday every 5 weeks
• Weekend schedule: Every 5th week, take 2 days off during the week and work Saturday and Sunday to perform patient admits.
• Dress code: No printed scrubs; black, grey, or khaki pants with polo or scrub top.

Requirements

• Minimum 1 year of experience in home health physical therapy.
• Required certifications:

  • Basic Life Support (BLS)
  • Doctor of Physical Therapy (DPT) degree
  • Valid State Physical Therapy License (Missouri)
• Locals accepted at a reduced rate.

Compliance & Documentation

• Background Checks:

  • National criminal check, state and county searches covering 10 years of all addresses and aliases.
  • Social Security Number trace with all aliases.
  • National Sex Offender check.
  • FACIS Level 3.
  • EPLS/GSA/SAM and HHS/OIG checks.
• Education Verification:

  • Official transcript or diploma with graduation date required.
• State Specific:

  • Missouri EDL Search required within 30 days of start.
• Licenses & Certifications:

  • Primary source verification of state license within 30 days of start.
  • NURSYS verification for RN, LPN/LVN, or AP if applicable.
• Health Documents:

  • Physical exam within last 12 months (not required annually).
  • Fit mask testing completed onsite.
  • Negative TB test or chest X-ray within 1 year, with annual updates.
  • Immunizations or declinations for:

    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug screening within 30 days of start covering multiple substances.
  • Seasonal flu vaccination or approved medical/religious exemption required during flu season (declinations not accepted).
• References & History:

  • Two professional references within 3 years of start.
  • Verification of most recent employer.
  • Skills checklist required.

Additional Notes

• Basic Dysrhythmia exam and medication competency exams may be required depending on profession/specialty.
• Orientation includes safety manual acknowledgement and agency nurse orientation quiz with a minimum passing score of 84%.
• Some certifications such as ACLS, PALS, TNCC, CPI, and NIH Stroke Certification may be required depending on unit or specialty (mostly for nursing roles).
• Candidates must review and sign confidentiality agreements and code of conduct documents prior to start.
