Position Details

• Shift: 4x9 hour day shifts (07:00 - 16:30)
• Experience Required: Minimum 1 year (first-timers accepted)
• Weekend Requirement: Work every third weekend
• On-Call: 1 primary night and 1 secondary night per week
• Floating: May float within scope as needed
• Schedule Flexibility: Block schedule requests can be discussed with manager
• Locals Accepted: Yes, no radius restrictions
• Return to Office (RTO) Restrictions: Ideally no more than 5 days

Background & Compliance Requirements

• Background Check within 30 days of start, covering all locations lived/worked in past 7 years
• EPLS/GSA/SAM check within 30 days prior to start
• HHS/OIG check within 30 days prior to start
• Illinois-specific searches including:

  • Department of Corrections Inmate Search
  • State Police Murderer and Violent Offender Against Youth Registry
  • Most Wanted Search
• Illinois Health Care Worker Registry Search (fingerprints must be taken in Illinois 3 business days prior to start)
• Education Verification (degree verification required)
• Competency Exam (required for all clinical/allied candidates)

Health & Safety Requirements

• Physical Exam within 1 year of start (must confirm good physical and mental health, free of communicable disease)
• Fit Mask test (annual expiration required for new contracts)
• Negative TB test or Chest X-Ray within 1 year
• TB Questionnaire (updated annually)
• Immunizations or proof of immunity (or signed declination) for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel Drug Screen within 30 days of start
• Seasonal Flu Vaccination or approved Medical/Religious Exemption required annually by November 30

Additional Requirements

• Photo for badge (color photo, mid-shoulders up, plain background) due before start
• Review and sign Code of Conduct within 7 days of contract signing
• Parking permit form to be completed if applicable
• References: 2 professional references required for clinical candidates
