Job Details
- Position: Radiation Therapist
- Floating: No floating between units

- Staffing Ratios: 2 therapists per each linear accelerator (linac)
- Preferred Experience: Elekta, Mosaiq, ExacTrac (preferred but not required)
- Charting System: Epic

Required Certifications
- ACLS (Advanced Cardiac Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- BLS (Basic Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- PALS (Pediatric Advanced Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- NRP (Neonatal Resuscitation Program) – Current certification by AAP, AHA, or Canadian Paediatric Society
- Chemotherapy Certification (if applicable)
- Electronic Fetal Monitoring (EFM) (if applicable)
- State Professional License – Copy required; CPDH certificate required for RTs working in California
- NURSYS Verification (for RNs)
- IV Certification (LVN/LPN/Other, if applicable)
- Management of Assaultive Behavior (MAB) (if applicable)
- NIH Stroke Certification (required every 2 years if applicable)
- CPI (Crisis Prevention Institute) (if applicable)
- Tech Certification Verification (for Allied Professionals)

Background and Compliance Requirements
- Background Check: Within 45 days of start, covering all locations lived/worked in past 7 years; must be updated annually. Includes SSN trace, Death Master Index, nationwide sex offender check, education verification, MVR (if home health), and state-specific abuse/neglect searches if applicable.
- E-Verify Attestation required.
- Education Verification: Required for all candidates; international education requires additional documentation.
- State-Specific Searches: Required for Washington and Oregon placements, including WSP/WATCH and OBRA Registry for CNAs in Washington.
- Drug Testing: 10-panel drug screen within 90 days of start; instant tests not accepted.
- References: Minimum of 2 supervisor references covering 1 year of employment within the last 3 years (6 months for CNAs).
- Employment Application: Must include at least last 5 years of employment with explanations for gaps or overlaps.
Health and Safety Requirements
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation.
- Fit Mask Test: Required for travel candidates starting on or after 2/5/2024; must be updated annually.
- Immunizations and Vaccines:
- Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) – 2 vaccines or positive titer; declination accepted
- Hepatitis B – vaccination series or signed declination; special rules for Alaska placements
- Varicella – 2 vaccines or positive titer; declination accepted
- Seasonal Flu Vaccine (required during flu season; declination accepted)
- COVID-19 Vaccine or Declination (required if onsite)
- Respiratory Safety Clearance: Required for all new travel starts on or after 2/5/2024; annual clearance for respirator use.
Competency and Skills
- Medication Test/Pharmacology Competency Exam: Required as applicable by profession.
- Color Blind Testing: May be required on a unit-by-unit basis.
- Competency Exams: Required as applicable by specialty.
- Skills Checklists: Required for applicable units and must be updated annually.
Facility Specific Requirements
- Floating Policy: Acknowledgement required; may be required to float to other Providence facilities within reasonable distance.
- Photo for Badge: Passport-style photo with neutral background required.
- Epic Training: Completion of Epic modules required prior to or shortly after start, depending on region and role.
- WorkRamp Modules: Completion of required pre-start, specialty, regional, and annual regulatory training modules via WorkRamp LMS platform. Modules vary by profession, specialty, and region.

- This is a travel/contract position at a short-term acute care hospital in Portland, Oregon.
