Required Credentials & Documentation
Background Checks
- 7-year background check within 6 months of start (including counties lived/worked, statewide, national sex offender, national criminal, OIG sanction, SAM.gov)
- EPLS/GSA/SAM, HHS/OIG, Sex Offender, OFAC Sanctions List searches completed within 30 days prior to start
- Education verification (3rd party) or diploma as placeholder if pending
- Transcript/Diploma upload required unless education verification is complete
Certifications
- ACLS (AHA only) – Required for many units including ICU, ED, PACU, L&D, and others
- BLS (AHA only) – Required for all RNs
- PALS (AHA only) – Required for select units including Pediatrics, ED, and Respiratory
- NRP – Required for Labor and Delivery, Mother Baby, NICU
- EFM (Electronic Fetal Monitoring) – Required for Labor and Delivery (AWHONN Advanced or NCC C-EFM within 2 years)
- Orientation Safety Manual Acknowledgement – Required
- Acknowledgement of Pass/Fail Testing – Required for all RN specialties
- State License Verification – Must be run within 30 days prior to start; no temporary or emergency licenses accepted without approval
- NURSYS Verification – For RNs and LPN/LVNs only, within 30 days prior to start
- Pass/Fail Testing (EKG/Medication) – Administered onsite; one attempt only; required for all RN specialties
Competency Exams
- Medication/Pharmacology Competency Exam (80% minimum)
- Dysrhythmia/EKG Competency Exam (80% minimum) for all RNs except certain units (e.g., Home Health, Same Day Surgery)
- Core Competency Exams Part I, II, and III (80% minimum, updated annually) covering topics such as:
- National Patient Safety
- HIPAA
- OSHA (Body Mechanics, Emergency Management, Environmental Safety, Fire Safety, Hazardous Chemicals)
- Infection Control/Bloodborne Pathogens
- Cultural Competence, Ethics, Patient Rights, Abuse, Pain Management, Restraints, Sexual Harassment, Workplace Violence
- Phlebotomy Competency Exam for PCT/PCA roles
Facility-Specific Requirements
- Completion of Self Learning Module & Post Test (100% score required) prior to start
- ECG Foundations for ECG Interpretation (Relias exam) required for all RNs to prepare for onsite pass/fail test
Health & Immunization Requirements (Must be completed within 72 hours of contract signing)
- Physical Exam or Statement of Health (within 12 months) signed by MD, DO, PA, or NP
- Fit Mask Test (N95 or equivalent) – annual expiration required for Aya candidates
- Tetanus/Diphtheria/Tdap (preferred, not required; declinations accepted)
- Negative TB test or Chest X-Ray (annual Quantiferon Gold or T-spot within 6 months, or 2-step PPD)
- Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B surface antibody titer or declination
- Negative 9 Panel Drug Test within 30 days of start (no rapid tests, no pending results)
- Seasonal Flu Vaccination (mandatory; no religious or medical declinations accepted)
- COVID-19 Vaccination (completed series and booster if eligible; no declinations accepted)
Employment History & References
- Two references required: one from a supervisor/manager within 12 months, second within 3 years
- Current and updated resume covering last 7 years of employment with explanations for gaps over 30 days
- Employment verification for 7 years required, must be completed 2 Fridays prior to start
- Unit-specific skills checklist required
This position requires strict adherence to credentialing, health, and competency standards to ensure compliance and patient safety.
