Description

Job Opportunity: Therapy/Rehabilitation - Travel/Contract

Location: Wichita Falls, Texas
Contract Length: 13 weeks (approximately 3 months)
Start Date: December preferred
Work Schedule: Facility will consider either 5x8 hour shifts or 4x10 hour shifts


Key Requirements and Documentation

Employment & Background Checks

  • Background check required within 1 year of start; includes Social Security trace, county searches for last 7 years, nationwide sex offender check.
  • EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start.
  • Background check must be renewed annually for contract workers.

Certifications (AHA Only unless otherwise noted)

  • ACLS (Advanced Cardiac Life Support) – Instructor, Provider, or RQI acceptable.
  • BLS (Basic Life Support) – Instructor, Provider, or RQI acceptable.
  • PALS (Pediatric Advanced Life Support) – Instructor, Provider, or RQI acceptable.
  • NRP (Neonatal Resuscitation Procedure) – Required as applicable.
  • CHEMO (Chemotherapy) – Required as applicable.
  • TNCC (Trauma Nursing Core Course) – Required as applicable.
  • EFM (Electronic Fetal Monitoring) – Required as applicable.
  • ENPC (Emergency Nursing Pediatric Course) – Required as applicable.
  • NIH Stroke Certification – Required as applicable.
  • Tech Cert Verification (National) – Required within 30 days of start.
  • Orientation Safety Manual Acknowledgement – Required and updated annually.
  • State License Verification – Required within 30 days of start.
  • NURSYS Verification (for RN, LPN/LVN, AP) – Required within 30 days of start.

Competency Exams

  • Medication Test/Pharmacology Competency Exam – Required as applicable.
  • General Competency Exam – Required as applicable.

Health & Immunization Requirements

  • Physical Exam within 1 year of start; must be completed within 72 hours of signing contract.
  • Tetanus/Diphtheria/Tdap vaccine within 10 years of start.
  • Negative TB test or Chest X-Ray within 1 year of start; 2-step PPD or equivalent required.
  • TB Questionnaire required for all candidates.
  • Rubeola (Measles): 2 vaccines or positive titer required; exemptions apply for those born before 1957.
  • Mumps: 2 vaccines or positive titer required; exemptions apply for those born before 1957.
  • Rubella: 2 vaccines or positive titer required; exemptions apply for those born before 1957.
  • Hepatitis B: Positive titer, series, or declination required.
  • Varicella: 2 vaccines, positive titer, or history of chickenpox required.
  • Drug Testing: Negative 9-panel drug screen within 1 year of start; must have plan for completion within 72 hours of contract signing.
  • Flu Seasonal Vaccination (2025-26): Required between October 1 and May 31; medical or religious exemptions allowed subject to facility review.
  • COVID-19 Vaccine: Preferred but not required.

Facility-Specific Documents

  • Flu Medical or Religious Exemption forms if applicable.
  • Parking Policy Acknowledgment.
  • Various facility-specific forms related to health records, code of conduct, confidentiality, security agreements, and orientation must be completed prior to start.

Additional Requirements

  • Government-issued photo ID (Driver’s License or State ID).
  • I9 or attestation.
  • E-Verify or attestation.
  • Employment application or resume required.
  • Two references within 3 years of start.
  • Skills checklist updated annually.
