Travel Physical Therapist - Home Health
Location: Bedford, New Hampshire

Job Description
- Provide physical therapy services in a home health setting with home visits and periodic office meetings.
- Average patients per day: 5-6
- Patient load: 5-10 patients per day, equating to roughly 6-8 points (points system based on visit types and activities).
- Types of cases: Start of Care, Follow Ups
- Schedule: Monday to Friday, 8 AM to 5 PM; no weekend rotation or on-call required.
- Territory: Must be willing to travel up to 75 miles from home clinic, covering areas including Manchester, Londonderry, Litchfield, Pelham, and Hudson.
- Weekly travel: Approximately 200-250 miles.
- EMR System: HCHB (Homecare Homebase)
- Productivity expectations: 30 units per week.
- Reliable transportation and current driver's license required.
- Mileage reimbursed at IRS standard rate (patient home to patient home travel compensated; commute to/from first and last patient is not billable).
- Locals accepted at the same rate.
- Must have a minimum of 1 full year of recent adult home health experience.
- First-time home health therapists are not accepted.
- CPR and BLS certification required.
- Pending licenses are generally not accepted unless the license is expected within a few days.
- Some positions may have a Baylor shift schedule (4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
Requirements & Qualifications
- Minimum 1 year recent adult home health experience required.
- Current, valid physical therapy license for New Hampshire.
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Reliable transportation and valid driver's license.
- Must carry agency-required liability insurance.
- Ability to travel within a 75-mile radius of the home clinic.
Compliance & Documentation
Candidates must complete and maintain the following before and during the assignment:
Licensure & Background
- Valid PT license in hand (pending licenses generally not accepted).
- Standard background checks including 7-year history, SS trace, county searches, nationwide checks, sex offender registry, motor vehicle record, and state-specific checks as applicable.
- Fingerprinting as required by state regulations (varies by state).
- Education verification (degree verification or transcript/diploma).
- References: Two professional references required.
- Work history verification: At least two most recent employers or last 7 years if only one employer.
Certifications
- CPR and BLS current certification.
Health & Immunizations
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella (Chickenpox) as per Aya standards.
- Drug screening: 10-panel drug screen required.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.

Additional Notes
- Candidates must sign new supplier packets at the start of each assignment and after any break in service, even if only one day. Annual completion required if working more than one consecutive year.
- Some state-specific requirements and forms may apply depending on the assignment location.
