Job Description
- Home Health RN Team 4
Required Certifications
- ACLS (Advanced Cardiac Life Support): AHA only
- BLS (Basic Life Support): Required for all RNs, AHA only
- PALS (Pediatric Advanced Life Support): AHA only, required for select units
- NRP (Neonatal Resuscitation Procedure): Required for Labor and Delivery, Mother Baby, NICU
- EFM (Electronic Fetal Monitoring): Required for Labor and Delivery (AWHONN Advanced or NCC C-EFM within 2 years)
- State License Verification: Must be current and verified within 30 days prior to start; no temporary or emergency licenses unless approved
- NURSYS Verification: For RNs and LPN/LVNs only, within 30 days prior to start
- Pass/Fail Testing: Required onsite for all RN specialties (includes Telemetry and Medication exams)
- Orientation Safety Manual Acknowledgement: Required
- Acknowledgement of Pass/Fail Testing: Required for all RN specialties
Required Competencies
- Medication Test/Pharmacology Competency Exam: Minimum 80% score required
- Color Blind Testing: Ishihara test within 1 year of assignment start
- Competency Exams: Must score at least 80% for relevant departments
- Dysrhythmia/EKG Competency Exam: Required for all RNs with 80% pass rate (exemptions apply for Home Health and certain units)
- Core Competency Exams (Parts I, II, III): Required annually with 80% passing score; covers National Patient Safety, HIPAA, OSHA topics, Infection Control, Ethics, Patient Rights, and more
- Phlebotomy Competency Exam: Required for PCT/PCA roles
Required Background Checks and Verifications
- 7-year background check within 6 months of start, including:
- Counties Lived/Worked
- Statewide and National Criminal Checks
- National Sex Offender Registry
- OIG Sanction Search
- SAM.gov and EPLS/GSA checks
- Education Verification (3rd party) of highest level of education; diploma may be used as placeholder if pending
- Sex Offender Search including all aliases
- OFAC Sanctions List Search
- Transcript/Diploma upload required unless education verification is complete
- Employment Verification for last 7 years, completed 2 Fridays prior to start; no pending verifications allowed
- Two references required (one from supervisor/manager within 12 months, second within 3 years)
Required Health Documents (Must be completed within 72 hours of contract signing)
- Physical Exam or Statement of Health (signed by MD, DO, PA, NP) confirming good health and no physical limitations
- Fit Mask Test (N95 or equivalent) required annually for clinical positions
- Tetanus/Diphtheria/Tdap vaccination (preferred, not required; declinations accepted)
- Negative TB test or Chest X-Ray (annual Quantiferon Gold or T-spot within 6 months, or 2-step PPD)
- Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B surface antibody titer or declination
- Negative 9 Panel Drug Test within 30 days of start (no rapid tests, no pending results)
- Seasonal Flu Vaccination (mandatory, no religious or medical declinations accepted)
- COVID-19 Vaccination: Completed series with booster if eligible; no declinations or partial series accepted
Additional Requirements
- Completion of facility-specific self-learning modules and post-tests (100% passing score required)
- Completion of ECG Foundations exam for ECG interpretation preparation (not required for clearance)
- Unit-specific skills checklist
- Acknowledgement of pass/fail testing policies
- Compliance with all facility and system policies related to employment and credentialing
