Description

Physical Therapist - Home Health

Location: Ketchikan, Alaska
Specialty: Therapy/Rehabilitation
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: Monday to Friday, 5x8 hour days (8:00 AM - 4:30 PM, start time flexible)
  • Schedule: Day position, no weekends or on-call required
  • Work Setting: Community-based, patient’s home with office on campus
  • Average Daily Census: 26 patients
  • Guaranteed Hours: 40 hours per week
  • Floating: May float to hospice within scope if needed; no floating to sister facilities
  • Locals Accepted: No
  • Reliable Vehicle: Required

Experience & Licensure Requirements

  • Minimum 1 year of Home Health / Older Adult experience required
  • 1 year Epic experience preferred
  • Alaska Physical Therapist License required (Compact PT license accepted)
  • Basic Life Support (BLS) certification required
  • First-time travelers accepted if they have OASIS experience
  • Pending state license not accepted

  • Congestive Heart Failure (CHF)
  • Orthopedic procedures including joint replacements
  • Neurological diseases

Job Responsibilities

  • Evaluate patients and develop treatment plans and goals
  • Provide treatment and modify plans as needed
  • Complete documentation accurately and timely per regulatory and licensure standards
  • Collaborate with healthcare team members to ensure continuous patient care
  • May supervise therapy assistants, aides, volunteers, and provide clinical instruction to students
  • Participate in quality improvement, new service development, staff meetings, and educational activities

Education

  • Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program (or foreign equivalent)

Required Skills

  • Strong organizational, analytical, verbal, and written communication skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Ability to assess patient conditions and exercise independent judgment

Certifications & Compliance Requirements

  • Alaska PT License and BLS required upon hire
  • Background checks including Social Security Verification, EPLS/GSA/SAM, HHS/OIG required
  • Physical exam within 1 year of start
  • Fit mask test within 1 year (for onsite candidates)
  • Drug testing within 30 days of start
  • Immunizations or declinations required for:
    • Tetanus/Diphtheria/Tdap
    • MMR (Measles, Mumps, Rubella)
    • Varicella
    • Hepatitis B
    • Flu seasonal vaccination or approved declination (seasonal requirement)
  • Respiratory safety clearance required for onsite candidates
  • Orientation modules and quizzes must be completed prior to start with 100% score

Additional Notes

  • Must sign SSA release for verification
  • Orientation materials and policies (dress code, masking, personnel restrictions) must be reviewed prior to start
  • Candidate must complete facility-specific forms such as criminal disclosure, demographic form, and badge photo
  • Onsite N95 fit testing available if needed
  • No on-call or weekend requirements
