Job Opportunity: Physical Therapist - Home Health (Travel/Contract)
Location: Glen Carbon, Illinois

Job Details
- Shift: Monday to Friday, 8:00 AM - 4:00 PM (5x8 schedule)
- Daily Caseload: 5-6 units per day
- Experience Required: Minimum 1 year
- First-time Travelers: Possibly accepted after interview
- Certifications Required: BLS (Basic Life Support)
- On-call: No
- Floating: Not required or allowed
- Locals Accepted: Yes
- Pending State License: Not accepted
- Return to Office (RTO) Restrictions: No more than 7 days off; requests must be submitted and approved or adjusted by the facility
- Guaranteed Hours: 3 shifts guaranteed per 13-week contract (cancelled shifts must be of regular shift length)
Patient Population & Department
- Multidisciplinary team focused on geriatrics and middle-aged adults; no pediatric patients
- Patient diagnoses include orthopedic, cardiac, neurological, pulmonary, diabetes, and other conditions
- Emphasis on case management skills, independence, time management, organizational skills, patient-centered goals, supervising PTA, teamwork, professionalism, communication, flexibility, positive attitude, and delivering excellent quality care
Facility & Work Environment
- Charting system: Epic
- Orientation provided with a preceptor
- Large parking space available
- Scrub color provided
Compliance & Requirements
Licensure & Certifications
- Must have a valid Illinois state license (no pending licenses accepted)
- BLS certification required
- Other certifications may be required depending on unit (e.g., ACLS, PALS, CPI, etc.)—refer to unit-specific requirements
- Primary source verification of professional license required
Background Checks & Screening
- Criminal background check within 6 months required (7-year search including statewide and nationwide checks)
- EPLS/GSA/SAM and HHS/OIG checks required quarterly
- Education verification of highest related degree required before start
- Fingerprinting not required for Illinois Home Care but required for other facility types in Illinois
- State-specific exclusion and sanction list checks required for Illinois placements
- Drug testing required within 1 year of start and must remain valid throughout assignment
- MO-specific background checks required if applicable (not relevant for Illinois Home Care)
Health & Immunization Requirements
- Physical exam within 1 year of start required
- Fit mask testing (N95) required and must be renewed annually
- Tdap vaccine or declination required (expires every 10 years)
- Negative TB test or chest x-ray within 1 year required; history of positive TB requires additional documentation
- Measles, Mumps, Rubella, Varicella immunity or vaccination required (with exceptions for certain units)
- Hepatitis B declination required (vaccination not mandatory)
- Seasonal flu vaccination or approved exemption required during flu season (Sept-April)
Facility-Specific Documents & Policies
- HIPAA Acknowledgement, Latex Allergy Questionnaire, Confidentiality Form, Regulatory Compliance Education, Hand Hygiene Policy, Fingernail Policy, Dress Code Policy must be reviewed and signed prior to start
- Badge photo required (head and shoulders, white background, no obstructions)
- Auto insurance copy required for driving positions
- Employment application and professional references required
- Unit-specific skills checklist and competency exams required within 1 year of start
Additional Notes
- No weekend or holiday call required; approximately every 6th weekend and 2 holidays off
- Candidate must provide a government-issued driver's license
- Case management and supervisory skills expected, including supervising PTAs
- Positive attitude and flexibility are important for success in this role
This position offers a structured schedule with a focus on adult home health patients, requiring strong clinical and organizational skills within a multidisciplinary team environment.
