Physical Therapist - Rehabilitation Therapy (Contract)

Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks


Job Summary

The Physical Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education.


Key Responsibilities

  • Evaluate patients and establish plans of care tailored to home environment needs.
  • Coordinate care with other disciplines and obtain orders for additional services as needed.
  • Provide direct patient care based on established care plans and physician orders.
  • Apply evidence-based physical therapy guidelines, Medicare regulations, and Conditions of Participation during evaluations, re-evaluations, and treatments. Modify treatment frequency and duration as appropriate.
  • Master CMS-required OASIS scoring and collaborate with Quality team for necessary adjustments.
  • Understand insurance requirements related to Home Health.
  • Perform interventions allowed under the physical therapy practice act, including wound care when applicable.
  • Participate in multidisciplinary conferences to develop patient care plans and communicate effectively with physicians and staff.
  • Document all skilled services, adverse events, and communications in the medical record using point of care documentation.
  • Engage in Quality Assurance Performance Improvement initiatives.
  • Meet productivity standards and maintain accurate time logs and mileage records.
  • Communicate daily with scheduler and optimize travel routes for continuity of care.
  • Supervise Physical Therapist Assistants and CNAs as required.
  • Participate in care conferences and plan admissions, OASIS time points, and discharges.
  • Maintain physical therapy licensure and continuing education requirements.
  • Adhere to ethical standards as defined by the physical therapy licensure board and agency policies.
  • Participate in agency-provided educational opportunities.
  • Perform other duties as assigned.

Minimum Qualifications

  • Bachelor’s degree or higher from an accredited Physical Therapy program.
  • Current license to practice Physical Therapy in Montana.
  • Valid driver’s license and appropriate auto insurance.
  • Preferred: Two years of previous Physical Therapy experience.
  • BLS/BCLS certification required.

This position offers an opportunity to work in a home health setting providing comprehensive physical therapy services with a focus on patient-centered care and interdisciplinary collaboration.
