Required Certifications
- ACLS (Advanced Cardiac Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- BLS (Basic Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- PALS (Pediatric Advanced Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- NRP (Neonatal Resuscitation Program) – Current certification by AAP, AHA, or Canadian Pediatric Society
- CHEMO (Chemotherapy Certification) – Verify if required
- EFM (Electronic Fetal Monitoring) – If applicable, AWHONN or NCC-EFM certification
- IV Certification (LVN/LPN/Other) – Verify if required
- Management of Assaultive Behavior (MAB) – Verify if required
- NIH Stroke Certification – Required every 2 years if applicable
- CPI (Crisis Prevention Institute) – Verify if required
- State License – Must have a valid professional license applicable to the role
- NURSYS Verification – Required for RNs
- Tech Cert Verification (National) – For Allied Professionals as applicable
Required Background Checks & Verifications
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
- Education Verification (3rd party) for all candidates
- State-specific searches as applicable (e.g., Oregon Abuse/Neglect, Washington WSP/WATCH)
- Transcript/Diploma verification as needed
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty within last 3 years (6 months for CNAs)
- Employment Application covering at least last 5 years (7 years for APN/Subcontractors)
- Skills Checklists as applicable, updated annually
Health & Immunization Requirements
- Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
- Fit Mask test (travel only, updated annually)
- Tetanus/Diphtheria/Tdap vaccine within 10 years or declination
- Negative TB test or Chest X-Ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) – 2 vaccines or positive titer or declination
- Hepatitis B – vaccination series or signed declination (special rules for Alaska placements)
- Varicella – 2 vaccines or positive titer or declination
- 10 Panel Drug Screen within 90 days of start (no instant tests)
- Seasonal Flu Vaccine or declination during flu season
- COVID-19 Vaccine or declination (required if onsite)
- Respiratory Safety Clearance (travel only, annually)
Competency Requirements
- Medication Test/Pharmacology Competency Exam as applicable
- Color Blind Testing if required by unit (minimum 80% score)
- Competency Exams as applicable by specialty
- Fetal Heart Rate Monitoring Assessment or EFM certification for L&D RNs
Facility Specific Requirements
- Photo for badge (passport style with neutral background)
- Completion of Providence Health and Services WorkRamp modules (Core, Specialty, Regional, and Annual Regulatory) prior to start
- Epic training modules completion required (within a week of orientation)
- Floating Policy Acknowledgement (may be required to float to other Providence facilities nearby)

- This is an outpatient PT position; outpatient experience is mandatory.
- No on-call, weekend, or block schedule requirements.
- The facility uses Epic for charting.
- Parking is free.
