Description

Registered Nurse - Home Health (Travel/Contract)

Location: Bastrop, Texas (Floating between Kyle and Bastrop area)
Contract Length: 13 weeks


Job Overview

  • Position: RN - Home Health - Field RN
  • Employment Type: Travel/Contract
  • Schedule: Monday to Friday, 8 AM - 5 PM, with on-call rotation (must be open to 1 night of call during the week)
  • Average Patients per Day: 4-6
  • Points: Approximately 24-30 points per week (equivalent to 40 hours/week)
  • Duties:
    • Administer skilled nursing care in patients' homes
    • Complete charting on Homecare Homebase (HCHB) & Oasis while at patient homes
    • Coordinate interdisciplinary care and develop/oversee Plan of Care
    • Provide care primarily to geriatric population
    • Participate in on-call rotation and weekend coverage as needed
  • Travel: Must be willing to travel up to 50 miles outside home base
  • Locals accepted within a 50-mile radius

Requirements

  • Experience:
    • Minimum 1 year in Home Health nursing
    • Minimum 2 years overall clinical experience
    • First-time home health nurses accepted
  • Certifications:
    • Valid RN license in hand (no pending licenses accepted)
    • BLS required (AHA or American Red Cross, RQI accepted)
  • Skills:
    • Proficient in Homecare Homebase (HCHB) and Oasis charting systems (1 year preferred)
  • Other:
    • Non-smokers preferred due to patient sensitivity (COPD patients)
    • Must be able to complete charting during patient visits
    • Must be willing to participate in on-call rotation and occasional weekend shifts

Job Description

The RN is responsible for providing quality skilled nursing care to diverse patients in their homes. Responsibilities include comprehensive patient assessments, care coordination with patients, families, and interdisciplinary teams, certifying providers, and developing individualized Plans of Care.

Required Documentation & Compliance

  • Current Driver’s License/State ID
  • Background Checks (within 1 year):
    • Last 7 years of residency and work states
    • Social Security Trace
    • County Searches
    • Nationwide Sex Offender Check
    • FACIS Level 1
    • Texas State Driver’s Record
  • Annual Background Checks: EPLS/GSA/SAM, HHS/OIG
  • Certifications:
    • ACLS (AHA or ARC, RQI accepted)
    • BLS (required for all units)
    • PALS (AHA or ARC) if applicable
  • State License Verification (primary source online verification)
  • Competency Exams: Medication/Pharmacology and unit-specific competency exams
  • Facility Specific: Personal or Auto Insurance (copy required)
  • Health Documents:
    • Physical exam within 12 months of start (statement of good health)
    • Negative TB test or chest X-ray (PPD, Q-Gold, or T-Spot within 12 months; chest X-ray within 5 years if positive)
    • Immunizations or titers for Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B (declination accepted)
    • Drug screening (9 panel within 12 months)
    • Seasonal Flu vaccination or declination during flu season (Oct 1 - Mar 31)
  • References:
    • 2 professional references for travel, allied, per diem acute care
    • 1 professional reference for per diem non-acute care
  • Employment Application and Annual Skills Checklist required

  • The employer is a female-founded, privately owned home health and hospice company serving central Texas and surrounding areas.
  • Mileage reimbursement follows IRS standard rates.
