Registered Nurse Travel/Contract Position

Job Requirements

• ACLS and BLS certification required (AHA only, name on certification must match legal name)
• Background in electrophysiology required
• No on-call time; must remain on shift until electrophysiology procedures are complete for the day

Licensure & Certifications

• State nursing license verification required (primary source verification)
• NURSYS verification required for all RNs
• ACLS (Advanced Cardiac Life Support)
• BLS (Basic Life Support)
• Other certifications may be required depending on unit (e.g., PALS, NRP, NIH Stroke Certification, CPI)
• Education verification required; diploma or official transcripts may be requested
• Skills checklist and competency exams required annually (unit-specific)

Background Checks & Documentation

• 7-year criminal background check within one year of start, updated annually
• EPLS/GSA/SAM and HHS/OIG checks required
• Education verification matching specialty required
• State-specific abuse registry check (KY) if applicable
• Employment application and professional references (2 for clinical, 1 for non-clinical)

Health & Immunization Requirements

• Physical exam within 1 year of start (not required by facility but required by agency standards)
• Fit mask test required annually; failed fit tests require waiver and religious exemption documentation
• Tdap vaccine within 10 years (declination acceptable for non-clinical)
• Negative TB test or chest X-ray within 1 year, updated annually
• Baseline TB risk assessment within 90 days of start, updated annually
• Immunizations required:

  • 2 MMR vaccines or IgG titers (Measles, Mumps, Rubella)
  • Hepatitis B surface antibody titer, series, or declination
  • 2 Varicella vaccines or IgG titer
• Negative 9-panel urine drug screen within 90 days of start
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (Oct 1 - Mar 31)
• COVID-19 vaccination or approved medical/religious exemption required

Facility Specific Notes

• Badge photo required (shoulders up, solid light background)
• Respiratory fit test waiver must be completed and approved annually if applicable
• Religious exemption required for fit test waiver at this location
• Flu and COVID exemption forms must be completed if applicable; no facility approval needed if agency accepts

Additional Information

• Medication test/pharmacology competency exam required as applicable
• Color blindness testing required prior to start
• Competency exams required for clinical staff prior to start
• Performance evaluations completed by unit managers; not required prior to start
