Job Opportunity: Nursing (Case Management) - Oregon
Location: Hermiston, Oregon
Employment Type: Contract
Contract Length: 13 weeks
Job Description
As a nurse in this role, you will:
- Assess, develop, implement, and monitor comprehensive care plans through an interdisciplinary team approach involving patients and families.
- Evaluate patients' biophysical, psychosocial, environmental, functional, and financial status.
- Advocate for patients and families to secure scarce resources and coordinate necessary services.
- Act as a liaison between hospital, post-hospital care providers, and community health resources.
- Facilitate patient and family education before admission, during hospital stay, and after discharge.
- Ensure continuity of care and coordinate transitions between care settings (hospital to home, rehab, long-term care).
- Collaborate with insurance companies to obtain authorizations and evaluate medical necessity for treatments and hospital stays.
- Monitor length of stay and identify potential discharge barriers.
- Address barriers to care including financial, social, and logistical challenges.
- Maintain accurate case management records, document assessments, interventions, and outcomes.
- Participate in interdisciplinary care conferences and collaborate with physicians, therapists, social workers, and other healthcare team members.
- Support hospital mission, vision, values, policies, and procedures.
- Be flexible to float to other qualified departments as needed.
- Participate in required education programs related to quality and accreditation standards.

- Advocate for patient needs and preferences.
- Educate patients and families on diagnoses, treatments, and self-care.
- Connect patients with community resources and support services.
- Address barriers to care and facilitate referrals to specialized services.
- Maintain thorough documentation and track patient outcomes.
- Collaborate with interdisciplinary teams and specialists.

Qualifications
Education:
- Required: Graduate of an accredited school of medicine, social work, nursing, or related medical occupation.
- Preferred: Experience coordinating community resources for post-acute care; BSN/MSN.
Licenses/Certifications:
- Required: Active Oregon RN License
- Preferred: Case Management Certification
