Job Description

As a nurse in this role, you will:

• Assess, develop, implement, and monitor comprehensive care plans through an interdisciplinary team approach involving patients and families.
• Evaluate patients' biophysical, psychosocial, environmental, functional, and financial status.
• Advocate for patients and families to secure scarce resources and coordinate necessary services.
• Act as a liaison between hospital, post-hospital care providers, and community health resources.
• Facilitate patient and family education before admission, during hospital stay, and after discharge.
• Ensure continuity of care and coordinate transitions between care settings (hospital to home, rehab, long-term care).
• Collaborate with insurance companies to obtain authorizations and evaluate medical necessity for treatments and hospital stays.
• Monitor length of stay and identify potential discharge barriers.
• Address barriers to care including financial, social, and logistical challenges.
• Maintain accurate case management records, document assessments, interventions, and outcomes.
• Participate in interdisciplinary care conferences and collaborate with physicians, therapists, social workers, and other healthcare team members.
• Support hospital mission, vision, values, policies, and procedures.
• Be flexible to float to other qualified departments as needed.
• Participate in required education programs related to quality and accreditation standards.

Qualifications

Education:

• Required: Graduate of an accredited school of medicine, social work, nursing, or related medical occupation.
• Preferred: Experience coordinating community resources for post-acute care; BSN/MSN.

Licenses/Certifications:

• Required: Active Oregon RN License
• Preferred: Case Management Certification
