Requirements and Qualifications

Background Checks

• 7-year background check required within 1 year of start; updated annually.
• Must include SS trace, county searches for last 7 years of locations lived/worked/schooled, nationwide search, and sex offender registry.

Certifications (as applicable)

• ACLS (Advanced Cardiac Life Support) – ARC or AHA accepted; not required for CNAs.
• BLS (Basic Life Support) – ARC or AHA accepted.
• PALS (Pediatric Advanced Life Support) – ARC or AHA accepted.
• NRP (Neonatal Resuscitation Procedure) – as required.
• Tech Certification Verification – required as applicable.
• NIH Stroke Certification – required for ICU, IMC/IMCU, inpatient, Med Surg, Step Down RN positions; renewed annually.

Competencies

• Medication Test/Pharmacology Competency Exam – required as applicable.
• Color Blind Testing – required for dialysis/kidney center roles.
• Competency Exam – required as applicable to profession/specialty.

Facility Specific Documents

• Photo for badge (passport-style, shoulders up, no filters).
• Compliance checklist to be completed by agency representative.
• Confidentiality statement to be signed prior to start.
• Flu vaccine religious or medical exemption forms if applicable.
• Dosimeter request form (Imaging Department only; not required for MRI Tech).

Health Documents

• Physical exam within 1 year of start (not required annually for non-clinical).
• Negative QGold TB test within 1 year of start (no TSpot/PPD accepted); negative chest X-ray within 60 days of start.
• Immunizations or positive titers required for: Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• Drug testing: Negative 9-panel drug screen within 14 days before start; instant acceptable.
• Flu seasonal vaccination or approved medical/religious exemption required during flu season (October to March).

History and References

• Two professional references verified within 2 years of start.
• For inpatient RNs (MS, MS/TELE, IMC, Stepdown, ICU): one reference must be from most recent acute care assignment with performance comments.
• Skills checklist required annually.

Additional Notes

• Candidates must comply with all credentialing and health requirements prior to start.
• Flu vaccination or exemption documentation must be submitted by November 30 for working candidates.
• Background checks and credentialing requirements are strictly enforced to ensure compliance.
