Job Opportunity: Home Health Occupational Therapist (OT)

Location: Portland, Oregon
Specialty: Therapy/Rehabilitation

Contract Length: 13 weeks


Job Description

  • Provide skilled home health occupational therapy services in West Portland and surrounding areas.
  • Must have an Oregon OT license.
  • Start Date: ASAP
  • Shift: 8-hour day shifts (no 4x10 or 3x12 shifts available)
  • Patient Ratio: 1:1; may visit up to 6 patients per day with a standard of 30 productivity points.
  • Experience Required: Minimum 1 year of skilled home health experience.
  • First-time travelers accepted.
  • No weekend work required.
  • BCLS certification required.
  • No on-call duties.
  • No floating between units.
  • Block scheduling not accommodated.
  • Local candidates within 50-75 miles considered local.
  • Facility may call off up to one shift every two weeks.

Special Requirements

  • Must have own transportation.
  • Typical daily travel radius up to 60 miles (60-90 minutes total drive time).
  • COVID-19 primary series vaccination plus one booster required.
  • Experience with OASIS documentation required.

Facility & Work Environment

  • Type: Home Health
  • Attire: Business casual; scrubs allowed but not required (no jeans).
  • Charting: EPIC Remote Client (laptop), Rover app (phone), and OASIS.
  • Parking: Free
  • Mileage reimbursement at IRS standard rates.

Required Certifications & Licenses

  • Oregon OT License (if working in border units, may require Washington license as well).
  • BCLS (Basic Life Support).
  • COVID-19 vaccination (primary series + booster).
  • Other certifications may be required based on clinical track (e.g., ACLS, PALS) – verify with clinical leadership.

Background & Compliance Requirements

  • Background check within 45 days of start, covering all locations lived/worked in past 7 years.
  • Must include SSN trace, death master index, nationwide and national sex offender checks, motor vehicle record (MVR), and education verification.
  • Oregon Abuse/Neglect search if applicable.
  • State-specific searches for Oregon and Washington as required.
  • Drug screening: 10-panel drug screen within 90 days of start (no instant tests).
  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work.
  • Immunizations and health documents required:
    • Tetanus/Diphtheria/Tdap (within 10 years or declination)
    • Negative TB test or chest X-ray within 12 months
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Varicella vaccination or positive titer
    • Seasonal Flu vaccine or declination (required during flu season)
    • COVID-19 vaccine or declination
  • Respirator fit testing required for travel candidates (updated annually).

Training & Competency Requirements

  • Completion of Providence Health and Services WorkRamp modules prior to start, including:
    • Core modules for Oregon home health professionals.
    • Specialty-specific modules if applicable.
    • Annual regulatory modules.
  • Epic training modules completion required within one week of orientation.
  • Medication and competency exams as applicable by profession.

Additional Notes

  • References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty within the last 3 years.
  • Employment application covering at least last 5 years required.
  • Skills checklists may be required annually depending on unit.
  • Facility may require photo for badge.
  • Floating policy acknowledgment may be required (though floating is not expected for this role).
