Job Description

  • Occasional home care visits required.
  • Utilize the nursing process to assess patient and caregiver needs, providing quality care in accordance with evidence-based practice, State and Federal regulations, and Vermont State Board of Nursing standards.
  • Develop and communicate realistic plans of care, involving patients and caregivers in the process.
  • Serve as Case Manager for assigned patients, managing caseloads and visit times per agency productivity standards.
  • Complete OASIS documentation accurately and timely, staying current with documentation requirements.
  • Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective, quality care.
  • Maintain and update high-tech skills such as IV use and maintenance, wound VAC, and complex wound care; ensure infection control standards are met.
  • Collaborate with LPNs within their scope of practice, delegate tasks, review documentation, and act on recommendations.
  • Evaluate the effectiveness of care plans and patient responses to ensure quality care delivery.
  • Maintain accurate and up-to-date nursing care documentation for effective communication within the healthcare team.
  • Follow agency protocols for visit documentation and completion.
  • Provide patient and caregiver education on medication side effects, treatment options, discharge instructions, and reportable conditions.
  • Identify and document interdisciplinary patient needs (e.g., physical therapy, social work) and communicate with relevant disciplines promptly.
  • Complete required agency, State, and Federal forms timely, ensuring availability during visits.
  • Conduct visits efficiently, separating supervisory visits from routine visits as needed, and communicate scheduling information promptly.
  • Assess patient needs for Home Care Aide (HCA) services, develop and coordinate plans of care, and provide ongoing supervision.
  • Report patient incidents (infections, injuries) to Quality Improvement specialists timely.
  • Coordinate safe patient discharges and transfers, ensuring all paperwork and information comply with policies and regulations.
  • Coordinate patient care with physicians, providers, team members, and vendors to support the plan of care.
  • Participate in monthly peer review audits, providing and accepting constructive feedback professionally.
  • Schedule and facilitate case conferences with management and team members to ensure effective communication and documentation.
  • Advocate proactively for patient care issues to ensure care is sensitive to individual patient and family needs.
  • Participate actively in interdisciplinary team (IDT) meetings to develop and update individualized plans of care within five days of admission and every fifteen days thereafter.
  • Stay current on pain control, symptom management, and advanced nursing skills, ensuring infection control throughout care.
  • Provide ongoing patient and caregiver education to support informed decision-making.
