Required Certifications

• ACLS (AHA Provider, Instructor, or RQI acceptable)
• BLS (AHA Provider, Instructor, or RQI acceptable)
• PALS (AHA Provider, Instructor, or RQI acceptable)
• TNCC preferred
• NRP preferred

Background Checks & Verifications

• 7-year criminal background check (including SSN trace, counties lived/worked/school, FACIS L3, DOJ Sex Offender Search, Nationwide)
• EPLS/GSA/SAM check
• HHS/OIG check
• Education verification (highest level) with diploma or transcript on file
• Primary source verification of state license
• NURSYS verification for RN

Competency Requirements

• Medication Test/Pharmacology Competency Exam (as applicable)
• Competency Exam (as applicable)

Facility-Specific Requirements

• Passport-style photo for badge (solid background, time sensitive)
• Completion of confidentiality, contractual, compliance, privacy, and emergency contact forms (time sensitive)

Health Requirements

• Physical exam within 1 year of start
• TDAP vaccine within 10 years or declination accepted
• Negative TB test (PPD, QGold, or TSPOT) within 1 year; for positive responders, negative chest X-ray within 5 years and annual TB questionnaire required
• Annual TB questionnaire
• Immunity or vaccination for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (positive titer required, vaccine series alone not accepted)
  • Varicella
• Negative 12-panel drug screen within 90 days of start (not required annually)
• Seasonal flu vaccination or agency declination/exemption required during flu season (masking required during high transmission)
• Respiratory safety clearance within 1 year, required annually
