Job Description

• Position: Home Health Physical Therapist
• Schedule: Flexible, either 5 days a week (5x8 hours) or 4 days a week (4x10 hours)
• Call: Required 1 Sunday every 5 weeks
• Weekend Rotation: Every 5th week, take 2 days off during the week and work Saturday and Sunday to perform patient admits
• Coverage Area: 13 counties in Missouri (Andrew, Atchison, Buchanan, Clay, Clinton, Daviess, Dekalb, Gentry, Harrison, Holt, Platte, Worth)
• Travel: Rotating weekly to the furthest counties; fewer patients scheduled on high mileage/travel days
• Dress Code: No printed scrubs; black, grey, or khaki pants with polo or scrub top

Requirements

• Minimum 1 year of experience in Home Health Physical Therapy
• Required Certifications:

  • Basic Life Support (BLS)
  • Doctor of Physical Therapy (DPT) degree
  • Valid State License for Missouri
• Locals accepted at a reduced rate

Compliance & Documentation Requirements

• Background Checks:

  • National Criminal Check
  • State and County Searches (10 years, all addresses and aliases)
  • Social Security Number Trace
  • National Sex Offender Registry
  • EPLS/GSA/SAM Check
  • HHS/OIG Check
• Education Verification: Official transcript or diploma with graduation date
• State Specific: Missouri EDL Search within 30 days of start
• Licenses & Certifications:

  • Primary source verification of state license within 30 days of start
  • NURSYS verification (for RN, LPN/LVN, AP if applicable)
• Health Documents:

  • Physical exam within last 12 months (not required annually)
  • Fit mask testing completed onsite
  • Negative TB test or chest X-ray within 1 year, updated annually
  • Immunizations or proof of immunity/declination for:

    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Drug screening within 30 days of start (comprehensive panel)
  • Seasonal flu vaccination or approved medical/religious exemption (mask required if exempt)
• References & Work History:

  • Two professional references within 3 years
  • Verification of most recent employer
• Competency Exams: As applicable by profession (medication, pharmacology, dysrhythmia/EKG)
• Facility Specific:

  • Confidentiality Agreement and Code of Conduct acknowledgment
  • Orientation quiz with minimum passing score of 84%

Additional Notes

• Mileage and travel time are considered in patient scheduling to balance workload
• Candidates must adhere to dress code and facility policies
• Some certifications and testing may be required annually or prior to start depending on role and unit
