Registered Nurse - Home Health (Travel/Contract)
Location: Bangor, Maine

Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and periodic office meetings; reliable transportation and current driver's license required.
- Average patients per day: 5-8 (assigned 5-10 patients daily, equating to roughly 6-8 points per day).
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Must have a minimum of 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
- First-time travelers not accepted; recent specialty experience required.
- Weekend and on-call requirements vary; discuss during interview.
- Must be willing to travel up to 75 miles from home clinic; mileage reimbursed at IRS standard rate (patient-to-patient travel compensated, commute to/from first and last patient is not).
- Local candidates accepted at the same rate.
- Pending licenses not accepted; license must be in hand.
- Typical schedule: Monday to Friday, 8 AM to 5 PM; some positions may have a Baylor Shift schedule (4x10s, Friday-Monday).
- Charting required using HomeCare HomeBase (HCHB) and OASIS proficiency required for RNs.
- Expected weekly productivity: 25-30 units.
- Weekly travel: approximately 250+ miles.
Required Certifications and Experience
- Current CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
- Proficiency with HomeCare HomeBase (HCHB) within the last 6 months or willingness to retrain.
- Proficiency in OASIS for RNs.
Compliance and Documentation Requirements
- Valid Driver’s License and personal/auto insurance required.
- Background checks including 7-year history, SS trace, county, nationwide, sex offender, and state-specific searches as applicable.
- Drug screening (10-panel) required.
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination series or declination required.
- Flu vaccination or declination documentation required for the current season.
- Education verification and work history verification (at least 2 most recent employers or last 7 years).
- Two professional references required.

Additional Notes
- This is a replacement position covering the Skowhegan area.
- No weekend rotation or on-call required for this assignment.
- Must be comfortable with travel and home health environment.
- Dress code: Scrubs.
