Experience Requirements

Background & Compliance Requirements

• Government Issued Photo ID (Driver’s License or State ID)
• 7-Year Criminal Background Check within 3 months of start (including SSN Trace, National Criminal, Sex Offender, Education Verification)
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start and monthly thereafter
• Foreign Equivalency Education Evaluation if applicable
• Education Verification from primary source (no hard copy degrees)
• Work History Verification (at least 2 past employments verified)
• References: 2 references within 3 years (Aya candidates only)
• Employment Application and Skills Checklist (annual)

Health & Immunization Requirements (Must be completed within 5 days of contract signing)

• Physical Exam (within 1 year, updated annually)
• Fit Mask Test (within 1 year, updated annually)
• Negative TB test or Chest X-Ray (within 1 year) or TB Questionnaire
• Immunizations or positive titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (Surface AB positive titer or completed series)
  • Varicella
• Drug Screening (within 30 days of start)
• Flu Vaccination or signed declination during flu season (documentation deadline Dec 3)
• Eye Exam (Snellen, within 1 year)
• COVID vaccination copy preferred but not required

State-Specific Requirements (New York)

• NYS Nurse Aide Registry (if applicable, within 3 months of start)
• NYS Criminal Background Check Fingerprint Results (for certain units)
• Disciplinary Action Search Results (for certain units)
• Sex Offender Registry Search (within 30 days of start)
• NY Medicaid Exclusion List check (within 3 months of start, monthly thereafter)
