Job Opportunity: RN Case Manager - Home Health

Location: Plainsboro and West Windsor (South team)
Contract Length: 13 weeks

Position Summary

Seeking an RN Case Manager to join the Home Health South team. This role involves managing patient care for Penn Medicine Home Health.

Requirements & Qualifications

• Valid RN license (wallet size)
• Current BLS certification
• Must pass background check, drug screen, and physical exam
• Required vaccinations and health screenings including:

  • COVID-19 Vaccine
  • Influenza Vaccine
  • Hepatitis B
  • MMR
  • TdaP
  • Varicella
• Competency exam and skills checklist completion
• Driver’s license and car insurance required
• Compliance with mandatory annual training and various verifications (employment, educational, license, references)
• Ability to pass color vision screening and fit mask test
• Must clear multiple screenings including FACIS 3M, OIG, SAM, OFAC sanctions, and sex offender registries
• TB screening and annual TB survey/questionnaire
